儿童髁突骨折临床治疗的近期疗效观察

目的:探讨儿童髁突骨折的治疗。方法:将32例2~12岁儿童髁突骨折患者,分为2~6岁组和7~12岁组,采用不同的方法进行治疗和功能锻炼。随访6个月,观察张口度、张口型和咬合关系。结果:张口度、张口型和咬合关系都恢复正常,无1例出现关节强直和开。结论:儿童期髁突骨折应当区分年龄段进行积极的治疗,3个月内被动功能锻炼和严密随访,可以防止关节强直等并发症的发生。

Short Term Observation on the Curative Effect of Non-surgical Treatment of Pediatric Condylar Fractures

Objective: To evaluate short term curative effect of non-surgical management of mandibular condyle fracture in children.Methods: 32 cases of pediatric condylar fractures were divided into two groups through age distribution.2~6 years(10 cases) in group A and 7~12 years(20 cases) in group B.Different treatments and functional exercises were employed and followed up for 6 months.8 cases in both groups accompanied with chin fractures were restored by internal fixation or stainless steel wiring.In group A,exercises of mouth opening 3~4 times a day were mentored by the physician or patients' parents lasted for 3 months.In group B,cases underwent intermaxillary elastic traction for 1 week,then practiced open mouth exercises for 3 months.Results: Mouth opening and occlusion were all recovered in two groups.No case of ankylosis or open bite was found.Conclusion: Pediatric condylar fracture should be treated differentially according to age variety of children.Early functional exercise within three months since injury and followed up are recommended to avoid the occurrence of ankylosis.Follow-up observations in a considerable period are necessary.
