川崎病中白细胞介素-4、血管性血友病因子及血管内皮生长因子的作用研究

摘    要:目的:探讨白细胞介素4(IL-4)、血管性血友病因子(vWF)和血管内皮生长因子(VEGF)在川崎病(KD)发病中的作用及其与冠状动脉病变(CAD)之间的关系。方法:采用ELISA法检测KD患儿23例血清IL-4、血浆vWF和VEGF的浓度,设正常对照组健康儿童,并与正常对照组、恢复期组、KD并发CAD者及无CAD者(non-CAD)之间进行比较。结果:①KD急性期组与恢复期组IL-4、vWF均高于对照组(P〈0.01);VEGF急性期组高于恢复期组(P〈0.01)。②KD急性期CAD者与急性期non-CAD者IL-4、vWF、VEGF均高于对照组(P〈0.01)。③KD恢复期CAD者、non-CAD者IL-4、vWF均高于对照组(P〈0.01);恢复期CAD与non-CAD者、对照组相比,VEGF的差异有统计学意义(P〈0.05)。结论:IL-4、vWF、VEGF均参与了KD的发生和发展过程,而IL-4、vWF与KD的免疫损伤、持续性血管炎有关,VEGF在一定程度上可预测冠状动脉的损害。

Abstract:Objective:To explore the role of IL-4,vWF and VEGF in the pathogenesis of Kawasaki disease (KD), and the relationship for the change of IL-4、vWF and VEGF with coronary artery disease (CAD) in KD children. Method:We dected the density of serum IL-4、plasma vWF and VEGF by ELISA in 23 KD patients, and compared with normal control group, dilatation of coronary arterygroup(CAD) and no dilatation of coronary artery (non-CAD). Result : ①The levels of IL-4 ,vWF of KD acute and early convalescence group were all higher than that of normal control group, VEGF levels of KD acute group were higher than that of early convalescence group. ②In KD acute CAD group and non-CAD, the levels of IL-4 .vWF and VEGF were all higher than that of normal control group (P〈0.01). ③In KD early convalescence group and non-CAD, the levels of IL- 4、vWF were all higher than that of control group (P〈0. 01), KD early convalescence CAD compared with non CAD and control group, VEGF levels were apparently changed (P〈0.05). Conclusion:All IL-4、vWF and VEGF take part in the occur rence of KD, IL-4、vWF connect with the immunity lesions and sustain vasculitis of KD, VEGF may forcast the coronary artery lesion in a certain degree.
