子宫肌瘤的MRI表现及其临床价值的分析

目的分析子宫肌瘤的MRI表现特点,评价MRI对该病的临床诊断价值.方法回顾性分析40例经手术病理证实的子宫平滑肌瘤患者的MRI资料,仔细观察壁间型、黏膜下型与浆膜下型肌瘤在TiWI,T2WI,STIR及增强后T1WI上的表现,并分别统计各型的例数,作行X列表的卡方检验.结果MRI对该病诊断的敏感性为100%,诊断符合率为90%;所有病灶T1WI均为等低信号,临近结合带无明显变化;子宫形态变化、病灶周边及内膜的改变与肌瘤的生长部位有关,经检验,差别有高度统计学意义(P＜0.01);各型在T2WI上信号的差别无统计意义(P＞0.05);T2WI及STIR图像对病灶的显示最清晰.结论选择合适的MRI检查序列与方法可以准确地将肌瘤的位置、大小及与周围的关系显示清楚,并能对病灶内部发生的病理改变作出一定的判断,对指导临床制定治疗方案、随访观察肿瘤的变化具有重要价值.

Clinical value of MRI for diagnosing the uterine leiomyoma

Objective To analyze MRI features of uterine leiomyoma and to ev al uate the diagnostic value.Methods MRI findings in 40 patients with surgically an d pathologically proved uterine leiomyoma were retrospectively analyzed,the-fin dings of three types which included intermural-,submucous-and subserous type o n T1WI,T2WI,STIR and enhanced T1WI were carefully observed and the number of eac h type was respectively calculated by using Chi-square test with line multiply schedule.Results The sensitivity of MRI in the diagnosis of uterine leiomyoma wa s 100 %,the coincidence rate of diagnosis was 90 %.The characteristic findings of all tumors were isointense or hypointense on T1WI.There was not significant change in Jiunctional zone.The change of uterine shape,peritumor and endometrium were related to the position of foci and the differences were significant(P< 0. 01) ,while on T2WI no statistical difference was found between three types(P > 0 .05).The lesions could be more clearly displayed on T2WI and STIR imagge.Conclus ion The optimal choise of MRI sequences and techniques are very necessary for cl early reveal the location,size and connection with surrounding tissue of uterine leiomyoma.
