高渗盐水雾化吸入联合气道廓清技术在慢性阻塞性肺疾病急性加重期患者排痰管理中的应用

摘    要:
目的 探讨高渗盐水雾化吸入联合气道廓清技术对慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者的排痰治疗效果。 方法 将90例AECOPD患者随机分为A、B、C组,A组采用0.9%氯化钠溶液联合沙丁胺醇进行雾化吸入,B组采用3%的氯化钠溶液联合沙丁胺醇进行雾化吸入,C组在B组的基础上联合应用气道廓清技术,比较3组患者排痰难度、呼吸困难指数、血气分析指标情况。 结果 干预3d和5d后,3组排痰难度、呼吸困难评分及PaO2和PaCO2比较,差异具有统计学意义,且C组的改善程度明显优于A组和B组。 结论 高渗盐水雾化吸入联合气道廓清技术能有效促进AECOPD患者痰液的排出,改善患者的缺氧症状,促进患者的康复。

Abstract:
Objective To explore the effect of hypertonic saline inhalation combined with the airway clearance on patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods Ninety AECOPD patients were randomly divided into group A, B and C. Group A was treated with the conventional inhalation of 0.9% sodium chloride solution combined with salbutamol inhalation. Both group B and C were treated with 3% sodium chloride solution and salbutamol inhalation, while group C was additionally given the airway clearance. The difficulty of expectoration, dyspnea index and blood gas analysis indexes were compared among the three groups. Results After 3 days and 5 days of intervention, there were significantly differences in the difficulty of expectoration and dyspnea among the three groups, with significantly bigger improvement of group C than group A and B. Moreover, the oxygen partial pressure and the partial pressure of carbon dioxide of the three groups were significantly improved, with that of group C significantly bigger than group A and B. Conclusion Hypertonic saline aerosol inhalation combined with the airway clearance can effectively promote the excretion of sputum in patients with AECOPD, relieve their symptoms of hypoxia and promote their recovery.
