胃癌组织中CD26的表达与临床病理特征及预后的相关性

目的探讨CD26的表达与胃癌临床病理学特征及预后的相关性。方法采用免疫组化EliVision Super法检测90例胃癌组织及相应90例癌旁正常胃黏膜组织、12例低级别上皮内瘤变和6例高级别上皮内瘤变组织中CD26蛋白的表达,分析其表达与临床病理学特征及预后的关系。结果CD26在胃癌肿瘤细胞中的阳性率为67.8%(61/90),在高级别上皮内瘤变组织中的阳性率为50%(3/6),两者差异无统计学意义(P>0.05);在低级别上皮内瘤变及癌旁正常胃黏膜腺上皮中均不表达;CD26表达与胃癌浸润深度(r=0.248,P=0.015)和pTNM分期(r=0.255,P=0.012)呈正相关。CD26在胃癌肿瘤浸润性淋巴细胞(tumor infiltrating lymphocytes,TILs)及间质细胞中均无表达。Kaplan-Meier及Log-rank单因素生存分析显示,CD26表达、肿瘤大小、浸润深度、远处转移和pTNM分期为胃癌预后的不良因素(P<0.05)。多因素Cox回归模型分析显示,肿瘤大小、远处转移和pTNM分期均为影响胃癌预后的独立危险因素(P<0.05),而肿瘤细胞中CD26表达、浸润深度并非预后的独立危险因素(P>0.05)。结论CD26的表达在胃癌的发生、进展中扮演重要角色,其可作为胃癌侵袭能力、远处转移、临床病理分期、早期诊断及负性预后因子的重要参考指标。

CD26 expression in gastric cancer tissues and its correlation with clinicopathological characteristics and prognosis

Purpose To investigate the expression of CD26 in gastric adenocarcinoma and correlation with clinicopathological characteristics and prognosis.Methods 90 resected samples of gastric adenocarcinoma tissues were collected as experimental group.12 cases of low grade gastric intraepithelial neoplasia tissues,6 cases of high grade gastric intraepithelial neoplasia tissues and 90 cases normal gastric tissues in 90 resected samples of gastric adenocarcinoma corresponding adjacent tissues were used as control groups.All specimens were detected by EliVision Super two-step immunohistochemistry method with the specific antibody of CD26.The relationship between expression of CD26 in gastric adenocarcinoma tissues and the clinicopathologic characteristics,as well as overall survival time in 90 cases cancerous patients was also analysed.Results The positive rates of CD26 was 67.8%in gastric cancers and 50%in high grade intraepithelial neoplasias,but there was no statistical significance between them(P>0.05).CD26 did not express in the low grade intraepithelial neoplasm and normal gastric tissues.Expression of CD26 was positively correlated with the depth of invasion and pTNM staging(P<0.05),but not with other clinicopathologic characteristics(P>0.05).Expression of CD26 was negative in tumor infiltrating lymphocytes and stromal cells of gastric adenocarcinoma tissues.Kaplan-Meier survival curves and Log-rank test results showed that the overall survival time were significantly related with CD26 expression,tumor diameter,depth of invasion,metastasis and pTNM staging(P<0.05).Multivariate Cox regression analysis showed that tumor diameter,metastasis and pTNM staging were independent prognostic risk factors for gastric cancer(P<0.05),but CD26 expression and depth of invasion were not independent prognostic risk factors for gastric cancer(P>0.05).Conclusions CD26 may play an important role and have a close correlation with the initiation and progression of gastric cancer.CD26 has potential to become an important referenced marker for estimating the ability of invasion,metastasis,pTNM staging,early diagnosis and poor prognosis in gastric cancer.
