rhGM-CSF凝胶与异种皮对深Ⅱ度烧伤创面愈合效果分析

摘    要:目的 探讨重组人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)凝胶与异种皮相对比,对深Ⅱ度烧伤创面愈合效果差异。方法 以入院病例号为编号,采用数字表法,将70例深Ⅱ度烧伤患者随机分成两组,每组35例。对照组按照常规治疗方法,同时削痂后给予异种皮(猪皮)覆盖;观察组患者削痂后给予rhGM-CSF凝胶覆盖创面。治疗7天时记录并观察患者换药疼痛程度、创面溶痂率、完全溶痂时间、创面愈合时间以及安全性。结果 观察组VAS和TESS分别为2.8±0.7分和5.3±0.8分,均低于对照组(P<0.05);第3、7、10天观察组创面溶痂率34.5%、64.6%、90.6%均优于对照组25.6%、48.7%、78.5%(P<0.05);观察组患者完全溶痂时间和创面愈合时间分别为8.3±3.8天和17.9±4.5天,优于对照组(P<0.05)。结论 与异种皮相比,rhGM-CSF凝胶可有效促进深Ⅱ度烧伤创面愈合,提高创面溶痂率,降低患者换药痛苦,安全性高,应用前景良好。

Abstract:Objective To explore rhGM CSF(recombinant human granulocyte macrophage set colony stimulating factor(G-CSF) gel with heterogeneous surface contrast, differences in the effect of deep second degree burn wound healing. Methods Seventy patients with deep second degree burn were randomly divided into two groups with 35 cases in each group. Control group according to the conventional treatment method and tangential excision after giving skin xenograft(pigskin) covering; patients of observation group tangential excision treated with rhGM-CSF gel to cover the wound. After 7d,the degree of wound dressing, wound healing rate, complete dissolution, wound healing time and safety were recorded and observed. Results The scores of VAS and TESS of observation group were 2.8±0.7 and 5.3±0.8, which was better than the control group(P<0.05).At the 3d, 7d, 10d,the wound healing rate of the observation group were 34.5%, 64.6%, 90.6%,respectively, which was better than the control group.In the observation group, the total dissolution time and wound healing time were 8.3±3.8d and 17.9±4.5d, respectively, better than the control group(P<0.05). Conclusion rhGM-CSF gel can effectively promote wound healing in deep second degree burn wounds, improve wound healing rate, reduce the pain of dressing change,It has high safety and good application prospect.
