胃镜下瘤内注射重组人p53腺病毒治疗晚期食道癌的安全性及短期疗效观察

目的探讨重组人p53腺病毒（rAd—p53）瘤内注射治疗晚期食道癌的安全性及临床可行性。方法2008年9月至2011年7月我院对11例晚期食道癌患者实施胃镜直视下瘤内注射rAd—p53,每周1次,每次1×10^12 VP,一个疗程为8周。结果治疗后2月,肿瘤缩小1例（9．09％）,无变化8例（72．73％）,增大2例（18．18％）;除自限性发热外,无明显不良反应。结论胃镜下瘤内注射rAd—p53短期无明显毒副作用,能较好地抑制局部肿瘤的发展,尤其对失去手术机会,不能耐受放化疗的患者,是一种可行的方法。

The safety and short-term clinical observation of endoscopic intratumoral injection of recombinant human p53 adenovirus on advanced esophageal cancer

To investigate the safety and Clinical efficacy of intratumoral injection of recombinant human P53 adenovirus(Gendicine) in patients with advanced esophageal cancer.11 advanced esophageal cancer patients of our hospital from September 2008 to July 2011 were treated with Gendicine by endoscopic intratumoral injection,the patients were intratumoral injected r-Ad P53 once weekly each at the dosage of 1 × 1011 VP,one course of treatment was eight weeks;After two months,the carcinoma size of 1 patients was decreased(9.1%),the carcinoma size of 8 patients were not changed(72.7%),and the carcinoma size of 2 patients were increased(18.2%).There were no adverse effect for all patients except self-limited fever.There were no obvious side effects by intratumoral injection r-Ad P53 under gastroscopy in patients with advanced esophageal cancer,and it can inhibit the development of local carcinoma.Especially it is a useful method for the patients who lose the chance of operation and can not tolerate chemotherapy and radiotherapy.
