摘    要:
目的探讨血清降钙素原(PCT)检测在判断急性中毒患者是否合并感染的临床价值。方法采用免疫荧光分析法检测《抗菌药物临床应用管理办法》实施后入院的54例急性中毒患者(实施后组)的PCT,并与管理办法实施前63例未检测PCT患者(实施前组)比较。结果两组年龄、体温、白细胞计数(WBC)、住院天数、死亡率比较,差异均无统计学意义(P>0.05);实施后组抗菌药使用率低于实施前组,差异有统计学意义(P<0.05);实施后组中使用抗菌药组PCT水平较未使用抗菌药组PCT水平增高,差异有统计学意义(P<0.05)。结论 PCT的高表达对细菌性感染的诊断及指导抗菌药的分级管理等均有较高的价值,有较好的应用前景,值得推广。

Abstract:
Objective To investigate clinical significance of detecting serum procalcitonin in acute poisoning patients with infection. Methods Serum procalcitonin (PCT) were measured by using a commercial fluroimmunoassay. The serum PCT levels in 54 acute poisoning patients who were admitted after implementation of "Administration rules of clinical use of antibiotics" (post-group) were compared to that in 63 pre-implementation patients (pre-group). Results No significant differences were found between two groups on age, body temperature, WBC count, days of hospital stay and mortality. However, the ratio of antibiotic usage in pre-group was significantly higher than that in post-group ( P 〈 0.05 ). Serum PCT levels in patients who were treated with antibiotics were significantly higher than that in patients who didn't use antibiotics ( P 〈0.05 ). Conclusion The detection of serum PCT is useful in diagnosis of bacterial infection in acute poisoning patients and may serve as an important reference for management of antibiotics usage.
