摘    要:
目的观察脑梗死急性期血清可溶性细胞间粘附分子-l(SICAM~1)的含量变化,探讨其在脑梗死发生发展过程中的作用.方法采用酶联免疫吸附法(ELISA)检测55例急性期脑梗死患者,32例其他疾病对照组和30例正常健康对照组的SICAM-1含量.结果(1)脑梗死患者SICAM-1含量较2对照组显著升高.(2)相关分析显示:脑梗死时SICAM-1含量与外周血白细胞数呈正相关,与神经功能缺损评分呈负相关,而与血脂水平无明显相关关系.(3)脑梗死患者SICAM-1含量与高血压史关系不密切,且SICAM-1含量与病灶大小无关.结论ICAM~1通过炎症机制参与了脑梗死的病理生理过程;急性期SICAM-1含量可以作为判断病情轻重的一个指标,脑梗死后应尽早使用抗ICAM-1抗体治疗.

Abstract:
Objective To explore the changes and the clinical significance of the levels of serum soluble intercellular adhesion molecule-1(SICAM-1)in patients with acute cerebral infarction. Methods Serum levels of SICAM-1were detected in55patients with acute cerebral infarction(within2days),32patients suffering fromother neurologic diseases(20patients with sciatica,12patients with trigeminal neuralgia)and30healthy persons by using ELISA. Results 1)The serumlevels of SICAM-1of the patients with acute cerebral infarction were significantly higher than those of the control groups.2)The serum levels of SICAM-1were positively correlative with the number of leukocytes at acute stage,but were negatively correlative with the European stroke scale,and there was no significant correlation between the serum levels of SICAM-1and the serum levels of cholesterol and triglyceride.3)The serum levels of SICAM-1in the patients of cerebral infarction with or without hypertension had not significant difference.There was no obvious correlation between the levels of SICAM-1and the size of infarct lesion. Conclusion The serum levels of SICAM-1were significantly increased in the patients with acute cerebral infarction and SICAM-1maybe participated in the pathophysiologic process through inflammatory mechanism.The serum level of SICAM-1could become a reliable index to estimate the clinical severity and the anti-ICAM-1antibody should be used as early as possible after cerebral infarction.
