经颅多普勒超声对昏迷患者的预后评估价值

目的探讨经颅多普勒超声（TCD）对评价重度颅脑损伤患者脑功能及预后判断的价值。方法对急性重度颅脑损伤患者进行动态TCD监测,并对频谱、血流速度、脉动指数等监测数据按国际标准进行评分、分级,分析其与患者预后的相关性。结果两组患者中外伤组分级与患者预后有良好的相关性,缺氧脑病组无明显关联性。结论TCD对判断病情、提示预后有重要意义,对脑外伤患者意义更大。

The evaluation of TCD to prognosis of coma patients

Objective To discuss the clinical diagnostic value of TCD to the prognosis of coma patiente. Methods Dynamic monitoring by TCD was used to acute severe craniocerebral injury patients and frequency spectrum,blood flow rate,and ripple index were graded,classed according to international standard,the correlation of the results and the prognosis were analyzed. Results In two subgroup,the grades of craniocerebral trauma one had good correlation to the prognosis,but in the oxygen deficiency one,there was no obvious correlation. Conclusion TCD has important meaning to the judgement of condition and prognosis. It has more clinical value in severe craniocerebral injury patients.
