老年患者上消化道出血临床特点及病因分析

目的探讨老年患者上消化道出血的病因及临床特点,以提高对本病的认识及诊治水平。方法回顾性分析2008年1月~2010年12月到我院就诊的老年患者上消化道出血118例的临床资料,并与同期非老年患者比较。结果呕血或排暗红色血便28例,占23.4%,黑便89例,占73.3%,低血压休克23例,占19.8%,其他25例,占20.9%。病因为十二指肠溃疡12例,占10%,胃溃疡40例,占34%,食管胃底静脉曲张破裂出血7例,占6%,胃癌21例,占18%,其它29例,占24.6%。结论老年患者上消化道出血除呕血、黑便外其它症状不典型,出血病因以消化性溃疡为主,与非老年组无明显差异。

The clinical features and causes of upper gastrointestinal bleeding in aged patients

Objective To explore the causes and clinical features of upper gastrointestinal bleeding in aged patients, and therefore to improve the understanding and the treatment level. Methods The clinical data of 118 aged patients with upper gastrointestinal bleeding being treated in our hospital from Jan 2008 to Dec 2010 were analyzed retrospectively. The aged patients were compared with homochronous young patients. Results There were 28 cases with hematemesis or bloody stools (23.4%),89 cases with the black stools(73.3%),23 cases with shock resulting from hypotension(19.8%),others were 25 cases(20.9%).Among them,12 patients were diagnosed as duodenal ulcer(10%),40 patients were diagnosed as gastric ulcer(34%),7 patients were diagnosed as esophageal and gastric fundus varices hemorrhage(6%),21 patients were diagnosed as cancers of the stomach(18%),others were 29 cases(24.6%). Conclusion Aged patients with upper gastrointestinal bleeding were characterized by hematemesis or black stools,the main causes of bleeding were peptic ulcer,the homochronous young patients had no significant difference.
