体外膜肺抢救危重症合并严重心肺功能衰竭的Meta分析

摘    要:目的:研究体外膜肺(ECMO)抢救危重症合并严重心肺功能衰竭的效果并予以Meta分析。方法:通过Cochrane系统评价方式,检索国内外核心数据库。纳入ECMO和传统机械通气治疗危重症合并严重心肺功能衰竭患者的随机对照试验,借助RevMan 5.2软件进行数据的分析。结果:与传统机械通气组相比,ECMO组可显著降低危重症合并严重心肺功能衰竭患者并发症发生率(RR=0.33,95%CI:0.21~0.46,P<0.01);且有利于降低危重症合并严重心肺功能衰竭患者病死率(RR=0.41,95%CI:0.29~0.58,P<0.01)。结论:ECMO抢救危重症合并严重心肺功能衰竭患者,可显著降低并发症发生率以及病死率。

Abstract:Objective:To study the effect of extracorporeal membrane lung(ECOM) in rescuing critical patients with severe cardiopulmonary failure and Meta analysis. Method:Through Cochrane systematic evaluation, search core databases at home and abroad. A randomized controlled trial of extracorporeal membrane lung and traditional mechanical ventilation in patients with critical illness complicated with severe cardiopulmonary failure was included, and data were analyzed with RevMan 5.2 software. Result:Compared with the traditional mechanical ventilation group, extracorporeal membrane lung group could significantly reduce the incidence of complications in patients with critical illness complicated with severe cardiopulmonary failure(RR=0.33, 95%CI=0.21-0.46, P<0.01).Compared with the traditional mechanical ventilation group, extracorporeal membrane lung group was beneficial to reduce the mortality rate of patients with severe cardiopulmonary failure(RR=0.41, 95%CI=0.29-0.58, P<0.01). Conclusion:Extracorporeal membrane lung can significantly reduce the incidence of complications and mortality in patients with severe cardiopulmonary failure. It is worthy of clinical application.
