腔内钬激光联合等离子电切术治疗尿道狭窄与闭锁疗效观察

目的探讨腔内钬激光联合等离子电切术治疗尿道狭窄及闭锁的疗效及安全性。方法对56例男性尿道狭窄与闭锁的患者采用输尿管镜下钬激光内切联合等离子电切术,观察术中、术后并发症及疗效。结果全部病例手术成功。手术时间20～80min,平均45min,出血10～80mL,平均30mL,留置尿管4～6周,拔尿管后排尿通畅,定期行尿扩。随访3～18个月,术后3、6和12个月最大尿流率（Qmax）、残余尿量（PVR）较术前明显改善,无严重并发症。结论腔内钬激光联合等离子电切术治疗尿道狭窄与闭锁,安全、创伤小、并发症少、近期疗效好,是治疗尿道狭窄及闭锁的理想方法。

Clinical observation of urethral stricture or atresia treated with transurethral thulium laser incision conbined plasma electrocision

【Objective】To investigate the efficiency and safety of treatment of urethral stricture or atresia with transurethral Holmium laser incision combined plasma electrocision. 【Methods】56 male patients with urethral stricture or atresia were exposed to the therapy of transurethral Holmium laser incision combined plasma electrocision with Ureteroscope, the curative effect and the complication in and after the operation were observed.【Results】All the cases were successful. The mean operation time was 45 minutes （20~80 minutes）, and the mean blood loss was 30 mL （10~80 mL）. With Indwelling Catheter for 4~6 weeks, all cases obtained unobstructed micturition after drawing the urinary catheter, and the patients were demanded to take the urinary expanding regularly. After follow up of 3 to 18 months, the maximum flow rate （Qmax） and residual urine volume （PVR） 3, 6 and 12 months after the operation were obviously im- proved than that of the pre-operation, and there was no severe complication.【Conclusion】Treatment of urethral stricture or atresia with transurethral Holmium laser incision combined plasma electrocision is safe, effective, and of mini- invasion and little complication, which is an ideal method to treat urethral stricture or atresia.
