212例糖尿病患者慢性大血管与微血管并发症5年随访

目的 :本文对北京地区 2 12例糖尿病患者进行 5年随访 ,以了解其糖尿病慢性并发症 ,包括大血管及微血管病变情况。方法 :分别对体重指数 (BMI)、血压、心电图、眼底、血脂、尿白蛋白排泄率(UAER)等进行分组检查。结果 :2 12例糖尿病患者中冠心病患病率由 1995年 8.0 %升高至 2 0 0 0年 14 .2 % (P >0 .0 5) ;脑卒中患病率由1995年 7.5%升高至 2 0 0 0年 8.5% (P >0 .0 5) ;糖尿病肾病由 1995年 2 9.2 %升高至 2 0 0 0年 4 7.2 % (P <0 .0 1) ;糖尿病视网膜患病率由 1995年 9.0 %升高至 2 0 0 0年 2 4 .5% (P <0 .0 1)。 2 12例糖尿病患者按血压 <14 0 /90mmHg和血压≥ 14 0 /90mmHg分组随访 ,发现其脑卒中、糖尿病肾病患病率二组间有显著性差异 (P <0 .0 5～ 0 .0 1)。结论 :糖尿病视网膜病变和糖尿病肾病的发生随病程延长明显增多 ,糖尿病时大血管并发症亦随病程延长有增加趋势。对此应早期进行干预治疗 ,尤其是降压治疗

Tracing for Chronic Macrovascular and Microvascular Complications in Patients with Diabetes After Five Years

To investigate the prevalence of diabetic chronic com plications including macrovascular and microvascular complications of 212 type 2 diabetic patients in Beijing in 1995 and in 2000.Method: Body mass index?blood pressure?ECG?retinal examinatio n were carried out in these patients together with the determination of plasma l ipid and urine albumin excretion.Results: The prevalence of coronary heart disease and stroke in 2000 in diabetic patients were 14.2% and 8.5% respectively,higher than those in 1995(8.0%?7.5%),but there was no statistical significance.The prevalence of dia betic retinopathy in 2000 increased from 29.2% to 47.2%( P<0.01 ).The prevalen ce of diabetic nephropathy in 2000 was 24.5%,significantly higher than that in 1 995(9.0%).The prevalence of diabetic nephropathy and stroke in people with blood pressure≥140/90 mmHg was significantly higher than those with blood pressure<1 40/90 mmHg,( P<0.05 ).Conclusion: Diabetic nephropathy and retinopathy increase with d uration.Early measure of prevention and treatment should be taken,such as anti hypertensive treatment.Diabetic macrovascular complications also increased with duration.
