颅脑血管外皮细胞瘤的CT、MRI与病理对照研究

摘    要:
目的:探讨颅脑血管外皮细胞瘤的临床、病理和CT、MRI特征。材料与方法:收集经CT和MRI诊断为脑膜瘤,经手术病理证实为血管外皮细胞瘤12例,年龄34 ̄58岁。CT检查7例,均采用增强前、后常规扫描。MRI检查12例,采用SE序列增强前、后扫描。结果:12例肿瘤全部发生在颅内脑外,7例CT平扫呈低等混合密度2例,呈等高混合密度5例;增强后扫描呈不均匀强化6例,均匀强化1例。骨窗显示病灶局部侵蚀性骨质破坏4例。MRI平扫,T1WI呈等高不均匀信号10例,呈等信号2例;T2WI呈不均匀等高信号10例,呈等信号2例。12例中7例显示脑膜尾征;增强后扫描1 均呈不均匀强化。结论:血管外皮细胞瘤具有一定的CT、MRI特征性表现:分叶状、丰富的血管流空、肿瘤内密度或信号不均匀、无肿瘤内钙化和骨质增生、局部颅骨呈溶骨性破坏。

Abstract:
Purpose: To analyse clinical. pathologic. CT and MRI features of intracranial hemangiopericytomas. Materials and Methods: 12 cases of intracranial hemangiopericytomas were proved by patholo- gy. Age ranged from 34 to 58 years. CT were performed in 7 Patients before and after intravenous contrast medium. MRI with and without contrast medium was available for 12 patients. Results: All patients were extraaxial tumors. Of 7 unenhanced CT scans, 5 hemangiopericytomas appeared as heterogeneously hyperdense tumors with focal areas of isodense. The other 2 tumors were hypo - isodense. of a total 7 contrast - enhanced CT scans, 6 hemangiopericytomas had heterogeneous enhancement; the other 1 had ho- mogeneous enhancement. Bone erosion was present in 4 of 7 hemangiopericytomas imaged with CT. None had hyperostosis or tumor calcificatins. On T1 - weighted MR images, 10 of 12 tumors were hyper- isosignal intensity with conical gray matter; the other 2 had isointense with cortical gray matter; On T2 - weighted images, 10 of 12 were heterogeneously iso - hyper signal intensity. The other 2 were isointense. All 12 tumors imged with contrast - enhanced T1 - weighted MR imaging showed hetero- geneously enhancement. 7 of the 12 had a "dural tail" sign. Conclusion: Intracranial Hemangiopericytomas has special CT and MRI features: multilobulated, rich in vascular voild flow in the tumor, hetero- geneous density or signal intensity on CT or MRI, hyperostosis and intratumoral calcification are not present, the tumor hat bone erosion.
