齐拉西酮和奋乃静对首发精神分裂症患者认知功能的影响

摘    要:目的探讨齐拉西酮和奋乃静对首发精神分裂症患者认知功能的影响。方法将100例首发精神分裂症患者随机均分为齐拉西酮组和奋乃静组,于治疗前和治疗12周后进行7个神经心理测验(共17项指标),评估患者的执行功能、注意力、言语学习和记忆以及分言语学习和记忆。结果治疗第12周末后,齐拉西酮组除Stroop C、数字广度测验-顺背、即刻再认和延迟再认外其他各项认知功能指标均有所好转,而奋乃静组共有7项指标恶化,包括即刻和延迟逻辑记忆、字色混淆测验CW、连线测验B、数字广度测验-倒背、即刻和延迟视觉再生。齐拉西酮组绝大多数认知功能测查结果均明显好于奋乃静组(P<0.05或P<0.01)。结论齐拉西酮对首发精神分裂症患者的认知功能有改善作用,而奋乃静对认知功能的某些方面有损害。

Influence of Ziprasidone and Perphenazine on Cognitive Function in Patients with First - Episode Schizophrenia

Abstract:Objective To compare the influence of ziprasidone and perphenazine on the cognitive function in first-episode schizophrenia patients. Methods A hundred patients with first-episode schizophrenia were randomly divided into ziprasidone group (n =50) and perphenazine group (n=50). The cognitive function of all the patients was blindly evaluated with 7 neuropsychological tests(17 items) including the executive function, attention, verbal learning and memory, visual memory before treatment and after 12-week treat-ment. Results After 12-week treatment, most of cognitive functions were improved to some extent in the ziprasidone group except Stroop-C, digit span subtest-forward, immediate visual recognition and delayed visual recognition, while 7 neuropsychological items were deteriorated in the perphenazine group, including immediate logical memory, delayed logical memory, trail making test-B, Stroop-CW, digit span subtest-backward, immediate visual reproduction and delayed visual reproduction. The ziprasidone group showed significantly better cognitive function in most of cognitive tests than the perphenazine group (P 〈0. 05 or P 〈0. 01). Conclusion Ziprasidone may improve the cognitive function in treating the patients with first-episode schizophrenia, while perphenazine may injury the cognitive function in some respects.
