~(14)C—尿素呼气试验筛选抗Hp治疗对象

建立小剂量~(14)-尿素呼气试验,结合胃镜、细菌培养和病理检查,对48例病人进行了研究。结果发现~(14)C-尿素呼气试验阳性者,胃镜下炎症程度及活动性明显高于阴性者。~(14)CO_2表达与胃粘膜炎症和Hp感染呈正相关。以~(14)CO_2值大于5000dpm作为筛选抗Hp感染临界值,所选病人经抗Hp治疗,症状、内镜下胃粘膜表现及Hp情况有明显疗效。提示,用~(14)C-尿素呼气试验可作为筛选抗Hp治疗指标之一,除避免了尿素酶试验,细菌培养和组织学检查受显色剂、培养条件及胃镜取材有局限性影响外,还解决了血清学不能确认Hp病人的问题,以~(14)CO_2值大于5000dpm选择治疗对象,可减少Hp耐药菌株和药物副作用的产生。

~(14)C-UREA BREATH TEST, A SIMPLE METHOD TO SCREEN PATIENTS NEEDING ANTI - HP THERAPY

It is known that helicobacter pylori (HP) infection is related to duodenal bulbar ulcer and chronic gastritis. But it is not known when the patients with HP infection need anti - HP treatment. We have developed a modified carbon- 14-urea breath test to screen the patients with HP infection who need anti -HP treatment. The results has been compared with those of bacterial culture and endoscopic biopsy in 48 patients. The results showed that the inflammationa! level and activity in positive patients are more serious than those in negative ones. The determined critical value of C -urea was above 5000 dpm to select subjects needing anti -HP treatment. The inflammation of mucous mem-brance deceased remarkably under the endoscopy after the treatment. Because this test is non -invasive , low cost and accurate in screening HP infectious patients, it can be applied routinely in clinic.
