超声对肝内外胆管结石的诊断价值分析

摘    要:目的 :评价超声诊断肝内外胆管结石的价值。方法 :对 1 6例经手术证实的肝内外胆管结石超声资料进行回顾性分析。结果 :超声诊断肝内外胆管结石的符合率为 84% ( 31 /37) ,声学特征为强或中强回声团后方伴声影 ,合并梗阻上游胆管的积液扩张。结论 :超声较易发现肝内外胆管结石 ,但位于结石梗阻下游以及胆总管十二指肠段的结石 ,因无胆汁作背景或气体干扰易漏诊 ,应与肝实质回声对比 ,进行仔细全面的探查 ,对于胆总管结石 ,可根据原发与继发的声学特征对重点区域仔细扫查 ,以期给予临床提供较为客观的诊断 ,选择更恰当的术式 ,减少残石率 ,以达到最佳的治疗效果。

Abstract:Objective:To evaluate the diagnostic value of ultrasound in stones of intrahepatic and extrahepatic bile ducts. Met hods:The data of 16 patients who were diagnosed as stones of intrahepatic and e xtrahepatic bile ducts by ultrasonography were reviewed and the echosonographic features were analyzed. All of 16 cases were confirmed by operation. Results:Th e diagnostic coincidence in stones of intrahepatic and extrahepatic bile ducts u sing ultrasound was 84% (31/37) .The key echosonographic features were intensive and middle intensive echo with the acoustic shadow in bile duct, and the duct a bove place of obstruction was dilated. Coclusions:The stones of intrahepatic an d extrahepatic bile ducts can be detected easily by ultrasonography. Sometimes t he stone failed be shown, if the stone was located the lower at the end of the e xtrahepatic bile duct, because of shortness of bilary or interference of air. So while examining stone of common bile duct one should be careful, and compared with echo of liver.
