经上皮准分子激光角膜切削术与准分子激光上皮瓣下角膜磨镶术治疗中高度近视的疗效比较

摘    要:
目的比较经上皮准分子激光角膜切削术(Transepithelial photorefractive keratectomy,Trans-PRK)与准分子激光上皮瓣下角膜磨镶术(Laser-assisted subepithelial keratomileusis,LASEK)治疗中高度近视的临床效果。方法 2017年5月至2019年1月,中高度近视患者110例159只眼,根据手术方式不同分为对照组患者55例78只眼,采用LASEK手术治疗,波长193 nm,治疗功率810 mW;观察组患者55例81只眼,采用Trans-PRK手术治疗,波长193 nm,治疗功率810 mW。随访3个月,采用视觉模拟疼痛评分(visual analogue pain score,VAS)比较两组患者术后1、3和5 d的改变;观察术前和术后3个月最佳矫正视力、裸眼视力及角膜光密度,记录术后感染、角膜上皮下雾状浑浊、眼压升高、散光、角膜穿透等并发症发生情况。结果术后3个月,两组患者最佳矫正视力、裸眼视力及角膜光密度较术前均有改善(P<0. 05),且观察组最...

Comparison between Trans-Epithelial Photorefractive Keratectomy and Laser-Assisted Subepithelial Keratectomy in Therapeutic Effect on Moderate and High Myopia

Abstract:
ObjectiveTo compare the clinical effects of transepithelial photorefractive keratectomy(Trans-PRK)and laser-assisted subepithelial keratomileusis(LASEK)in the treatment of moderate to high myopia.MethodsTotally 110 patients with 159 eyes with moderate to high myopia from May 2017 to Jan.2019 were divided into the control group and observation group by surgical methods.The 55 patients with 78 eyes were treated with the LASEK having a wavelength of193 nm and an output of 8~10 mW.The 55 patients with 81 eyes in the observation group were given Trans-PRK surgery(wavelength:193 nm;output power:8~10 mW).After 3 months’follow-up,the visual analogue pain score(VAS)was used to compare the changes of the two groups before and 1,3 and 5 days after the operation.The best corrected visual acuity was observed before and 3 months after the surgery.Uncorrected visual acuity and corneal optical density were recorded for complications such as postoperative infection,corneal epithelial haze,elevated intraocular pressure,astigmatism and corneal penetration.ResultsCompared with preoperative data,the best corrected visual acuity,naked eye visual acuity and corneal optical density were improved 3 months after the operation(P<0.05).The best corrected visual acuity in the observation group was lower than that in the control group(P<0.05),and the naked eye visual acuity and corneal optical density higher than those in the control group(P<0.05).The VAS scores of the observation group were lower than those of the control group(P<0.05)1,3 and 5 days after the operation.The incidence of postoperative infection,corneal epithelial haze,elevated intraocular pressure,astigmatism and corneal penetration was 3.64%in the observation group,which was significantly lower than that(14.6%)in the control group(P<0.05).Conclusion Compared with LASEK,Trans-PRK,a safer therapy with less pain,is worthy of clinical application as it is more conducive to the recovery of early visual acuity in patients with moderate to high myopia and to the improvement of their corneal optical density.
