肝硬化死亡原因24例分析

目的　对临床晚期肝硬化患者的死因进行回顾性分析 ,以提高对该病防治的认识。方法　回顾性分析、总结肝硬化死亡患者的临床资料。结果　本组收集肝硬化患者 3 80例 ,住院死亡 2 4例 ,住院病死率 6.3 3 %,死亡原因中以并发消化道出血最多见 ( 17/2 4,70 .8%) ,其次为肝性脑病 ( 14 /2 4,5 8.3 %)和感染 ( 9/2 4,3 7.5 %) ,肝肾综合征 ( 7/2 4,2 9.2 %) ,电解质紊乱 ( 5 /2 4,2 0 .8%) ,肝癌 ( 3 /2 4,12 .5 %)。合并 2个或 2个以上并发症 17例 ( 70 .8%) ,其中以合并消化道出血和肝性脑病为主 ( 11/17,64 .7%)。结论　肝硬化死因中以综合因素导致者为多 ,其中以消化道出血为诱因诱发其他并发症者最多见。救治大出血、预防再出血是降低肝硬化病死率的关键

Retrospective analysis of death causes in 24 patients with liver cirrhosis

Objective Death causes of patient with liver cirrhosis were analyzed in order to improve its prevention and treatment.Methods The clinical data of the dead patients with cirrhosis of liver were analyzed retrospectively. Results 380 cases were collected,in which 24 patients were dead.The mortality rate was 6.33%(24/380).Gastrointestinal hemorrhage was its main cause (17/24,70.8%),successively,hepatic encephalopathy(14/24,58.3%),infection(9/24,37.5%),hepatorenal syndrome (7/27,29.2%) and electrolyte disorder ,Three patients had liver carcinoma (5/24,12.5%).17 cases had merged two or more complications (70.8%).Conclusion The dead causes are mainly attributed to synthetic factors.Gastrointestinal hemorrhage is the main inducement that leads to other complications.Management of acutevairceal hemorrhea and prevent-ment of rebleeding are the key to reduce mortality of the paitents with liver cirrhosis.
