彩色多普勒血流显像在脾血管应用解剖中的意义

目的探讨彩色多普勒血流显像（color Doppler flow imaging,CDFI）在脾血管显像中的作用,确立其在脾脏微创手术中的意义。方法随机选取实施腹腔镜脾切除术患者20例,术前行CDFI探测脾蒂终末血管走行类型,脾血管走行与胰腺关系,脾动、静脉管径;按照不同解剖类型采取相应的手术方式,并记录术中实际脾血管各项解剖指标,与CDFI结果对比,进行统计处理。结果所有20例患者均顺利完成手术,CDFI检测的各项解剖指标与术中实际结果基本符合,无统计学差异（P〉0.05）。结论利用CDFI可明确脾血管的解剖类型,从而指导脾脏微创手术实施,增加手术安全性。

Significance of CDFI in Applied Anatomy of Splenic Blood Vessels

Objective To explore the effect of CDFI in visualization of splenic blood vessels and to definite its importance in microinvasive splenic operation.Methods Twenty cases requiring laparoscopic splenectomy(LS)were selected randomly.The anatomic course type of splenic vessel pedicle,the adjacent relation between splenic vessels and pancreas,and the diameters of splenic artery and vein were detected on CDFI before operation.The different operation methods were adopted according to the different anatomic types.The indices above mentioned were recorded again on telerecording in LS and were compared with indices on CDFI.Results All of LS were applied successfully.There were no difference between the indices on CDFI and those on telerecording(P>0.05).Conclusions We can identify the anatomic types of splenic blood vessels on CDFI,which may be used to instruct microinvasive operation for spleen and to increase the LS safety.
