磁敏感加权成像对脑微出血的诊断价值

摘    要:目的 探讨磁敏感加权成像(SWI)对脑微出血的诊断价值。方法 对266例临床拟诊脑血管病病人行常规MR颅脑平扫T2WI、T1WI、T2FLAIR、DWI和SWI序列,对各序列显示的微出血数量及敏感度进行比较。结果 59例病人脑实质内有微出血,SWI序列共检出微出血病灶305个。T1WI序列检出微出血灶49个,病灶检出敏感度为16.1%;T2WI序列检出微出血灶79个,病灶检出敏感度为26.1%;T2FLAIR共检出微出血灶99个,病灶检出敏感度为32.3%;DWI检出微出血灶45个,病灶检出敏感度为14.5%。各序列病灶检出敏感度比较,差异有统计学意义(χ2=147.40-242.71,P〈0.05)。结论 SWI序列对微出血的检出具有高度的敏感性,使得早期发现并评价微出血成为可能,对脑卒中的治疗具有重要的指导价值。

DIAGNOSTIC VALUE OF MAGNETIC SWI IN CEREBRAL MICROHEMORRHAGE

Abstract:Objective To assess the value of susceptibility weighted imaging (SWI) in the diagnosis of cerebral microhemorrhage (CMH). Methods Conventional MR plain scanning (T2WI, T1WI, T2FLAIR, DWI sequences and SWI sequences) was conducted in 266 patients with a primary diagnosis of cerebral vascular disease (CVD), the number of microhemorrhage (MH) and detection sensitivity was compared among the sequences. Results Fifty-nine patients were confirmed with intracerebral MH, of which 305 MH lesions were detected by SWI, and 49 by T1WI sequence, with a detection sensitivity of 16.1%; Seventy-nine MH lesions were detected by T2WI, the lesion detection sensitivity was 26.1%. Ninety-nine MH lesions were detected by T2FLAIR, with a sensitivity of 32.3%, and 45 MH lesions were detected by DWI, the sensitivity being 14.5%; the differences of lesion-detection-sensitivity among the sequences were statistically significant (χ2=147.40-242.71,P〈0.05).
Conclusion SWI sequence has a high sensitivity in detecting microhemorrhage, making the early discovery and evaluation of microhemorrhage become possible, the method has a considerable value in guiding therapy for cerebral stroke.
