加巴喷丁对乳癌根治术患者术后疼痛、焦虑和睡眠质量的影响

【目的】探讨加巴喷丁对乳癌根治术患者术后疼痛、焦虑和睡眠质量的影响。【方法】64例ASAⅠ～Ⅱ级的择期乳癌根治术患者，随机分为加巴喷丁组（G组）和安慰剂对照组（P组），每组32例。G组：手术前夜和术前2h口服加巴喷丁1200mg；P组：手术前夜和术前2h口服安慰剂。术后2h，6h，12h，24h，48h进行视觉模拟评分。此外，采用状态特质焦虑量表对患者手术前夜以及术后6h，24h，48h的状态焦虑水平评枯。采用匹兹堡睡眠质量指数量表对患者前1个月、手术前后的睡眠质量进行评分。【结果】两组患者术后48h内血压、心率、呼吸、脉搏氧饱和度均稳定，两组间副反应差异无统计学意义。与P组相比较，G组的睡眠质量提高和焦虑评分明显降低。【结论】加巴喷丁能够明显减轻乳癌根治术患者术后疼痛、焦虑和改善睡眠质量，并不增加不良反应的发生率。

Effect of Gabapentin on Postoperative Pain,Anxiety and Sleep Quality in Patients with Breast Cancer

[Objective]To investigate the effect of gabapentin on postoperative analgesia, anxiety and sleep quality in patients with breast cancer. [Methods] Sixty four patients （ASA Ⅰ or Ⅱ ） undergoing breast surgery were randomly allocated to two groups with 32 cases for each. Group G received 1200mg oral gabapentin at night and 2h before surgery. Group P received placebo orally. In the postoperative period, pain was assessed by using visual analogue scale （VAS） at 2h., 6h, 12h, 24h and 48h. State anxiety was measured at night before surgery and 6h, 24h, 48h after the operation by using State-Trait Anxiety Inventory （STAI）. Sleep quality was measured by PSQI. [Results]There was no significant difference in adverse effects and the postopera- tive mean arterial pressure, heart rate, respiratory rate values and pulse oxygen between two groups. Postop- erative anxiety scores were less in the group G than those in placebo group. In addition, VAS pain scores and PSQI scores were significantly reduced in group G. [Conclusion] Gabapentin can improve sleep quality, postoperative anxiety and analgesia after breast surgery without increasing adverse effects.
