综合性医院内科门诊神经症性症状病人对医院满意度及其影响因素研究

摘    要:目的了解综合性医院内科门诊病人神经症性症状的发生率和具有此类症状病人对医院满意度,并分析对医院满意度的影响因素,提出护理对策。方法从某3家医院内科门诊随机抽取502例病人,用Zung焦虑自评量表,Wakefield抑郁自评量表,Albert惊恐障碍和躯体化障碍评估手册,一般情况调查表评估研究对象,调查伴或不伴神经症性症状的发生率,并调查神经症性症状病人对医院满意度,最后分析神经症性症状病人对医院满意度的影响因素。结果334例(66.5%)为躯体疾病不伴有神经症性症状,168例(33.5%)为有神经症性症状,其中神经症的58例,躯体化障碍的14例,躯体疾病伴神经症性症状的96例;神经症性症状组医院满意度显著低于躯体疾病不伴有神经症性症状组;神经症性症状病人对医院满意度的影响因素,作用由大到小依次为特殊检查次数、自认疾病程度、累积就诊次数、累积医疗费用、神经症性症状、药物治疗。结论综合性医院内科门诊病人的心理问题不容忽视,应加强门诊护士的精神卫生服务意识和观念、重视对精神卫生知识的学习和运用等,以对此类病人进行合理干预。

Abstract:Objective To investigate the prevalence of neurotic symptoms in internal medicine department of general hospital and outpatients' satisfactory rate towards hospital, analyze factors affecting satisfactory rate and put forward nursing strategies. Methods 502 patients, selected from outpatients in internal medicine department of three general hospitals, were investigated with SAS, SDS, Albert Manual for The Panic and Summarization Scales and the general questionnaire. The prevalence of neurotic symptoms in internal medicine department and patients' satisfactory rate towards hospitals were also surveyed. Finally, factors affecting satisfactory rate of patients with neurotic symptoms towards hospital were analyzed. Results 334 patients (66.5%) in internal medicine department had physical diseases without having neurotic symptoms and 168 patients (33.5%) had neurotic symptoms, including 58 patients with neurosis, 14 with summarization disorder, and 96 with physical diseases complicated with neurotic symptoms. Satisfactory rate towards hospital of group of patients with neurotic symptom was much lower than that of group of patients with physical disease only. According to the influencing degree, risk factors influencing satisfactory rate were times of special examination, personal knowledge on disease, aggregated times of treatment, aggregated medical cost, the degree of neurotic symptom, and medical treatment in turn. Conclusion Internal medicine department outpatients' psychological problems can not be underestimated and should be given rational intervention by enhancing mental hygiene service consciousness and conception and attaching importance to learning and application of mental hygiene knowledge.
