高血压患者颈动脉粥样硬化与其血清CD40/CD40L及MMP-9水平的关系

目的:分析高血压患者颈动脉粥样硬化(CAS)与其血清白细胞分化抗原40(CD40)、154(CD40L)及基质金属蛋白酶9(MMP-9)水平的关系。方法:采用彩色多普勒超声测定150例高血压患者颈动脉内膜中层厚度(CIMT),同时观察斑块数量及大小。根据硬化斑块指数(PI)分级(结合CIMT、斑块数量)将高血压患者分为PI 0~3级组;根据CIMT增厚程度将高血压患者分为CIMT正常组(11例)、CIMT增厚组(30例)及CIMT斑块组(109例)。50例体检健康者为对照组。ELISA测定各组血清CD40、CD40L及MMP-9水平,分析高血压患者CIMT、PI与其血清CD40、CD40L及MMP-9水平的相关性。结果:各组高血压患者血清CD40、CD40L和MMP-9水平均显著高于对照组(P0.05),且与患者CIMT增厚程度及PI分级呈正相关,与CIMT增厚程度相关系数分别为0.989、0.996、0.992(P均0.05),与PI分级间相关系数分别为0.983、0.991、0.985(P均0.05)。结论:CAS高血压患者血清CD40、CD40L和MMP-9水平存在不同程度升高,有效调控其浓度可能是CAS的防治方法之一。

Relationship between the Level of CD40/CD40L and MMP-9 in the Patients of Hypertension with Carotid Acherosderosis

Objective:To invistigate the relationship between the serum level of CD40,CD40Land MMP-9and carotid otherosclerosis(CAS)in the patients of hypertension.Method:The carotid intima-media thickness(CIMT)of 150cases patients with hypertension were measured by color Doppler ultrasound,Plaque index of patients with hypertension was divided into 0to 3level based on CIMT,plaque volume and plaque size,according to the thickening degree of CIMT,patients with hypertension were divided into normal group(11cases),thickening of the group(30cases)and plaque group(109cases);The serum CD40,CD40Land MMP-9level were measured by ELISA.To study the correlation of CIMT,PI and serum CD40,CD40Land MMP-9levels.Results:The serum level of CD40,CD40Land MMP-9in different group with hypertensive patients were significantly higher than the control group(P<0.05),and it was positively correlated with the thickening degree and PI levels of the CIMT with patients,the correlation coefficients were 0.989,0.996and 0.992 with thickening degree,the correlation coefficients were 0.983,0.991and 0.985with PI.Conclusion:The varying degrees of serum CD40,CD40Land MMP-9 were existed in hypertension patients with CAS.It may be a kind of method of the prevention and treatment of CAS by regulate the serum level of CD40,CD40Land MMP-9.
