宫腔细胞学检查筛查子宫内膜病变的可行性探讨

目的: 探讨应用子宫内膜细胞采集器采集子宫内膜细胞,进行宫腔细胞学检查,筛查子宫内膜病变的可行性及临床应用价值。 方法: 2004年6月～2004年10月于我院肿瘤门诊就诊的有症状或存在高危因素的50例病人,同时进行宫腔细胞学检查和分段诊断性刮宫术,比较两种检查方法的取材满意率、诊断结果、出血量及疼痛感觉等方面的差异。 结果: 应用宫腔细胞学检查和分段诊断性刮宫术,在筛查子宫内膜病变方面,在取材满意率、诊断结果符合率方面无显著性差异,在出血量、疼痛感觉方面有显著性差异。宫腔细胞学检查在诊断子宫内膜癌及诊断子宫内膜良性病变方面分别具有81.8%、78.6%的敏感性以及97.2%、93.9%的特异性。 结论: 应用子宫内膜细胞采集器采集子宫内膜细胞,进行宫腔细胞学检查,筛查子宫内膜病变是完全可行的,并且具有简单方便、易于操作、出血量少、几乎无痛等优越性,适用于症状人群及高危人群的常规筛查,在子宫内膜癌的筛查方面起到了很好的预警作用。

Clinical Value of Endometrial Cytopathological Examination in the Diagnosis of Endometrial Lesions

Objective: : To evaluate the feasibility of endometrial cytopathological examination as a screening methodfor the diagnosis of endometrial lesions. Methods : From July to October of 2004, tubal aspiration specimensfrom 50 patients at high risk for developing endometrial cancer were examined cytopathologically. Specimensobtained through dilatation and curettage were histopathologically examined. Blood loss and pain were also evaluated during the procedures. Results : No significant difference was found in coincidence rate and samplingsuccess rate between endometrial cytopathological examination and dilatation and curettage, but there was asignificant difference in blood loss and pain when comparing these two procedures. Endometrial cytopathological examination demonstrated a sensitivity of 81.8% for endometrial cancer and 78.6% for benign endometrial lesions. The specificity of endometrial cytopathological examination was 97.2% for endometrial cancer and93.9% for benign endometrial lesions. Conclusion : Endometrial cytopathological examination is a simple, safeand reliable screening method for the diagnosis of endometrial lesions.
