Tim3在慢阻肺疾病患者外周血中的表达及其与疾病转归的相关性

目的探讨慢性阻塞性肺疾病患者外周血单个核细胞(peripheral blood mononuclear cells, PBMCs)中T细胞免疫球蛋白黏蛋白分子3(Tim3)的表达及其与疾病转归的相关性。方法选取2018年1月至2019年6月在干休所就诊的慢阻肺患者63例以及同期健康体检者26例,慢阻肺患者按照临床诊断标准分为轻度组、中度组和重度组。实时荧光定量PCR测定慢阻肺患者治疗前后外周血PBMCs细胞中Tim3的表达,并采用受试者工作特征曲线(ROC)评价Tim3表达水平在慢阻肺患者治疗疗效预测中的价值。结果与健康体检者比较,轻度患者组Tim3表达维持在较低水平,中度和重度患者组其表达明显升高(P<0.05)。治疗后Tim3表达水平也随病情缓解而降低。Tim3预测慢阻肺患者治疗疗效的曲线下面积(AUC)为0.694 (P=0.016),灵敏度为80.0%,特异度为75.0%,截断值(cut off)为0.324ng/mL。结论慢阻肺患者外周血PBMCs细胞高表达Tim3,且随病情严重程度而升高。Tim3可作为判断慢阻肺患者病情好转或恶化的参考指标。

Expression of TIM-3 in PBMCs of COPD patients and its association with progress of the disease

Objective To investigate the expression of T cell immunoglobulin domain and mucin domain-3(TIM-3)in peripheral blood of patients with COPD,and to explore its relationship with prognosis.Methods 63 patients with COPD treated in sanatorium from January 2018 to June 2019 were divided into the mild,moderate and severe groups according to clinical diagnosis and classification criteria.Another 26 healthy volunteers during the same period were enrolled in the control group.The mRNA level of TIM-3 was measured by real-time fluorescence quantitative PCR before and after treatment.The value of TIM-3 expression level in the prediction of therapeutic effect in patients with COPD was evaluated by receiver operating characteristic curve(ROC).Results Compared with healthy volunteers,the expression of TIM-3Tim3 was maintained at a low level in mild patients and significantly increased in moderate and severe patients(P<0.05),and the TIM-3 level of COPD patients was downward after effective therapy.The AUC of Tim3 was 0.694(P=0.016),the sensitivity was 80%,the specificity was 75%,and the cut off value was 0.324ng/mL.Conclusion TIM-3 is highly expressed in PBMCs cells in peripheral blood of patients with COPD and increases with the severity of the disease.TIM-3 could be used as a reference index to judge the improvement or deterioration of patients with COPD.
