CRP、CEA及CA125表达水平与非小细胞肺癌患者行VATS术后疗效及预后的相关性

目的探讨C反应蛋白(C reactive protein,CRP)、癌胚抗原(carcino embryonic antigen,CEA)及癌抗原125(carbohydrate antigen 125,CA125)表达水平与肺癌患者行电视辅助胸腔镜手术(VATS)术后疗效及预后的相关性。方法选取2014年1月-2016年1月我院收治的128例肺癌VATS患者以及同期100例健康人为研究对象,分别将其作为研究组和对照组。根据研究组患者血清CRP、CEA及CA125表达水平从低到高分为低表达和高表达组各64例,对所有患者进行VATS术,观察手术疗效及CRP、CEA及CA125表达水平与VATS术后疗效及预后的关系。结果研究组肺癌患者术前血清CRP、CEA及CA125表达水平显著高于健康对照组,差异具有统计学意义(P<0.001);肺癌患者血清CRP、CEA及CA125表达水平与患者性别、年龄无关,与患者TNM分期、病理类型、以及分化程度存在显著相关性(P均<0.05);单因素分析结果显示临床分期、分化程度、CRP、CEA以及CA125表达水平高低与肺癌患者预后存在显著相关性(P<0.05)。COX回归分析结果显示临床分期、分化程度、CA125表达水平高低是影响肺癌患者术后预后的独立因素;CRP、CEA及CA125高表达组患者的术中出血量、术后置管时长及引流量、住院时长,相比同指标低水平表达组均明显增加(P<0.05)。CRP、CEA及CA125低表达组患者的中位无进展生存期分别为31.2、29.8、32.6个月,明显高于高表达组(22.6、21.3、23.6个月),差异均具有统计学意义(P<0.05)。结论肺癌患者血清中CRP、CEA及CA125表达水平与VATS疗效和预后存在显著相关性,CRP、CEA及CA125呈低水平表达患者的中位生存期相比高水平表达患者显著延长。

Correlation between the expression levels of CRP,CEA and CA125 and the efficacy and prognosis of patients with non-small cell lung cancer after VATS

Objective To investigate the relationship between the expression levels of C-reactive protein,carcinoembryonic antigen and cancer antigen 125 and the efficacy and prognosis of patients with lung cancer undergoing video-assisted thoracoscopic surgery(VATS).Methods 128 patients with VATS who were admitted to our hospital from January 2014 to January 2016 and 100 healthy subjects during the same period were selected as the study group and the control group.According to the serum CRP,CEA and CA125 levels,the study group was sub-divided into the low expression group and the high expression group,64 cases in each group.VATS was performed in all patients,and the the relationship between surgical outcome and CRP,CEA and CA125 expression levels and efficacy and prognosis after VATS were observed.Results The expression levels of serum CRP,CEA and CA125 in the study group were significantly higher than those in the control group(P<0.001).The expression levels of serum CRP,CEA and CA125 in lung cancer patients were not related to gender and age,but they were significantly correlated with TNM stage,pathological type and differentiation degree(P<0.05).Univariate analysis showed that the clinical stage,differentiation,CRP,CEA and CA125 expression levels were significantly correlated with the prognosis of lung cancer patients(P<0.05),and the results of COX regression analysis showed that the clinical stage,degree of differentiation,and level of CA125 expression were independent factors affecting the prognosis of patients with lung cancer.The intraoperative blood loss,postoperative catheterization time and drainage volume,and duration of hospital stay in the high expression groups were significantly higher than those in the low expression group(P<0.05).The median progression-free survival of the low expression group was 31.2,29.8,and 32.6 months,respectively,which was significantly higher than that of the high expression group(22.6,21.3,and 23.6 months)(P<0.05).Conclusion There is significant correlation between the expression of CRP,CEA and CA125 and the efficacy of VATS and prognosis of lung cancer patients,and the median survival time of patients with low expression of CRP,CEA and CA125 is significantly longer than that of patients with high expression of CRP,CEA and CA125.
