早期胃癌的内镜诊断及病理分析

目的分析早期胃癌的内镜下表现肢病理特点，以提高早期胃癌的诊断率。方法回顾分析我院2001—2006年经手术证实为早期胃癌的26例患者的临床及病理资料。其中男性19例，女性7例，术前均有不同程度腹部不适病史3个，1～2年。结果26例早期胃癌患荇术前均川胃镜检查加病理活检确诊为胃癌，胃内分布以胃窦最多，13例（50％）；病理以管状腺癌最多，12例（46．2％）；病灶周同组织示萎缩性肖炎11例（42.3％），肠上皮化生10例（38．5％），中重度不典型增生8例（30．8％），幽门螺旋杆菌阳性23例（88．5％）。结论胃窦部是早期胃癌好发部位．病理以管状腺癌为主，癌前病变及Hp与早期胃癌关系密切，胃镜加病理活检是早期胃癌确诊的主要手段。

Analysis of the pathology characteristic and endoscopic diagnosis in early gastric cancer

Objective To analyze the characteristic of gastroscope and pathology in early gastric cancer(EGC)for promoting the rate of diagnosis.Methods Review and analyse clinical and pathological date of 26 cases proved being EGC through operation in our hospital between 2001 and 2006.Among them,19 cases were male and 7 cases were female.All patients have 3 months to 2 years history of abdominal discomfort to different extent.Results All 26 patients suffering from EGC were confirmed being gastric cancer through examination of gastroscope before operation,the distribution of which within stomach mainly located at gastric antrum(13 case,50%),moreover,pathology indicated tubular gland cancer was the most(12 case,46.2%),tissue around focus indicated atrophic gastritis accounted for 11 cases(42.3%),intestinal metaplasia occupied 10 case(38.5%),middle and severe atypical proliferation were 8 case(30.8%),23 cases(88.5%)were positive with helicobacter pylori(Hp).Conclusions Gastric antrum is the most place where EGC occurred.Tubular adenoma is the most pathological type.Lesion precancerous and HP have tight relation with gastric cancer.Gastroscope plus pathology biopsy is a main means to diagnosis EGC.
