彩色多普勒超声对急性睾丸疼痛的鉴别诊断价值

摘    要:目的分析彩色多普勒超声对急性睾丸疼痛的鉴别诊断价值。方法回顾性分析240例急性睾丸疼痛患者,其中7例睾丸扭转、205例急性附睾睾丸炎、38例泌尿系结石导致肾绞痛。对比分析不同原因导致的急性睾丸疼痛患者其彩色多普勒的表现和差异。结果 7例睾丸扭转患者彩色多普勒超声均未探及明确血流信号,精索内睾丸动脉频谱呈低速高阻型,而急性附睾睾丸炎显示睾丸及附睾血流丰富,动脉频谱呈高速低阻型,肾绞痛患者睾丸形态及血流无异常。结论彩色多普勒超声对急性睾丸疼痛的鉴别诊断有较高的临床意义,特别是彩色多普勒超声未探及明确血流信号或血流信号明显减弱对明确睾丸扭转有较高的临床价值。

The Value of Color Doppler Ultrasonography in the Differential Diagnosis of Acute Testicular Pain

Abstract:Objective To study the clinical value of Color Doppler ultrasonography in the differential diagnosis of acute testicular pain.Methods 240 cases of acute testicular pain patients were retrospectively analyzed,including 7 cases of testicular torsion,205 cases of acute epididymal orchitis,38 cases of urinary calculi leading to renal colic.Comparative analysis of the color Doppler ultrasonography in patients with acute testicular pain with different causes.Results 7 cases of testicular torsion were no blood flow signals in color Doppler ultrasound or low-speed high-resistance type of arterial spectrum,while the acute epididymal orchitis showed a rich blood flow,and high-speed low-resistance type of arterial spectrum.The testicular morphology and blood flow signal were normal in renal colic.Conclusion There was a high clinical significance of color Doppler ultrasonography in the differential diagnosis of acute testicular pain,especially no or less blood flow signals in color Doppler ultrasound for diagnosis of testicular torsion.
