半导体激光照射治疗PICC后静脉炎预后的影响因素

目的筛选影响半导体激光治疗经外周血管穿刺中心静脉置管（peripherally inserted central catheters,PICC）后机械性静脉炎预后的相关因素,为PICC置管后机械性静脉炎的治疗提供理论依据。方法回顾性分析了湖北省肿瘤医院2004年8月至2010年3月间半导体激光治疗PICC置管后机械性静脉炎273例的临床资料,对患者的年龄、性别、原发病、穿刺部位、穿刺血管、静脉炎分级、静脉炎发生部位、治疗期间空腹血糖及PICC置管前是否化疗等相关因素进行单因素分析,并进行Logistic逐步回归分析。结果单因素分析显示,患者的性别、静脉炎分期、置管前是否化疗等3项临床指标各组间差异有统计学意义（P〈0.05）;多因素Logistic逐步回归分析显示,静脉炎分期和置管前是否化疗两项是影响半导体激光治疗PICC置管后静脉炎预后的独立危险因素（P〈0.05）。结论静脉炎分期和置管前是否化疗是影响半导体激光治疗PICC置管后静脉炎预后的独立危险因素,分期高及置管前已化疗者临床预后较差。

Multiple Factor Analysis of Prognosis of Semiconductor-laser Treatment of Mechanical Phlebitis in Patients with Peripherally Inserted Central Catheter

Objective To investigate the major factors affecting the prognosis of semiconductor-laser treatment of mechanical phlebitis in the patients with peripherally inserted central catheters（PICC） so as to provide theoretical foundation for semiconductor-laser treatment of mechanical phlebitis in the patients with PICC.Methods The clinical data of 273 patients with semiconductor-laser treatment for mechanical phlebitis after PICC in Hubei Provincial Cancer Hospital between August 2004 and March 2010 was studied retrospectively.Prognostic factors were analyzed by univariate and logistic regression according to age,sex,primary disease,location and blood vessel of puncture,location and staging of phlebitis,WBC count,chemotherapy before PICC and fasting blood-glucose.Results Univariate analysis showed that there were significant differences in sex,staging of phlebitis and chemotherapy before PICC（P0.05）.Multivariate logistic regression analysis showed that staging of phlebitis and chemotherapy before PICC were two indepondent risk factors related to the prognosis of semiconductor-laser treatment of mechanical phlebitis in the patients with PICC（P0.05）.Conclusion Staging of phlebitis and chemotherapy before PICC are independent risk factors for the prognosis of semiconductor-laser treatment of mechanical phlebitis in patients with PICC.The prognosis of patients with more staging of phlebitis and chemotherapy before puncture are poorer.
