子宫内膜异位症术后应用曲普瑞林治疗的效果与护理

目的：探讨子宫内膜异位症（内异症）合并不孕症在腹腔镜手术＋曲普瑞林治疗后的妊娠、复发、疼痛缓解情况与护理。方法选择行腹腔镜手术治疗的内异症合并不孕患者91例，按随机数字表法同时依据患者意愿进行适当调整，将患者分为观察组39例，给予曲普瑞林治疗3个月，对照组52例不给予曲普瑞林治疗。两组均给予细致的围手术期护理。随访追踪1年，观察术后妊娠、复发及疼痛情况。结果观察组术后1年的自然妊娠率为25．6％（10/39），内异症复发率为5．13％（2/39）；与对照组术后1年的自然妊娠率（23．1％）和复发率（5．77％）相比差异无统计学意义（P＞0．05）；观察组疼痛缓解率为38．46％高于对照组13．46％，差异有统计学意义（χ2＝35．374，P＜0．01）。结论对于有生育要求的内异症腹腔镜手术治疗的患者，术后用曲普瑞林没有明显增加1年内的妊娠率，可考虑在术后积极试孕，同时加强医患沟通，加强对患者的心理护理。

Effect and nursing of Triptorclin therapy for postoperative endometriosis

Objective To study the pregnancy , recurrence , pain relieving and nursing for patients after＆amp;nbsp;laparoscopy and the effect of Triptorclin therapy for endometriosis .Methods 91 patients with endometriosis combined with infertility were included in this study , from January 2010 to June 2011 .According to the random number selection and considering patients ’ will, 39 patients were assigned as the treatment group , treated with Triptorclin for 3 months after laparoscopy therapy , while 52 patients without the treamtment of Triptorclin were at the control group .Both the groups were given meticulous perioperative nursing care .The pregnancy , recurrence and pain relieving were followed up after one year .Results The pregnancy and recurrent rate of treatment group was 25.6%（10/39） and 5.13%（2/39）, respectively; the pregnancy and recurrent rate of control group was 23.1% and 5.77%, respectively.There was no significant difference between treatment and control group （P＆gt;0.05）.The effect of pain relieving was significant , 38.46%patients had relieved pain in treatment group while there were 13.46%patients in control group （χ2 =35.374, P＆lt;0.01）.Conclusions Triptorclin cannot increase pregnancy rate for the endometriosis patients who are planning to pregnancy .For the postoperative patients, positive pregnancy method and smooth communication with doctors and nurses , as well as the psychosocial nursing , are of great benefits .
