腹腔动脉及其分支血管造影的形态学与临床意义

摘    要:目的:为指导介入治疗观察腹腔动脉及其分支血管造影的自然形态。方法:对500例因原发性肝癌等疾病行介入治疗的腹腔动脉、肠系膜上动脉或腹主动脉插管造影的DSA图象进行分析。结果:腹腔动脉开口96.4%位于T12~L1椎体平面,开口朝上占49.6%,水平开口25.6%,开口朝下24.8%。腹腔动脉呈典型分布占89.4%,变异10.6%。肝动脉呈典型分布占72.8%,变异27.2%。肝固有动脉走行与肝总动脉呈钝角62.4%,直角13.0%,锐角24.6%。胃十二指肠动脉与肝总动脉呈钝角82.4%,直角6.0%,锐角11.6%。脾动脉常见形态有上弧型、水平型、横S型、下弧形等,回旋型及波浪型少见占6.7%。结论:熟悉腹腔动脉及其分支血管造影的自然形态与变异,才能确保肝胆胰脾胃疾病介入治疗的准确性和有效性。

Abstract:Objective:To study natural morphology of angiography of celiac artery and its branches for directing intervention therapy. Methods:The pictures of digital substraction angiography (DSA) of celiac and superior mesenteric arteries or abdominal aorta (performed in 500 patients with hepatocellular carcinoma or other diseases) during intervention therapy were analyzed. Results;The opening of celiac artery was located at level of the twelfth thoracic vertebra to the first lumbar vertebra in 96.4 % . The opening toward the head was in 49.6 %. toward feet was in 24.8 % , and horizontally in 25.6 % . The distribution of celiac artery was showed as typical and variant in 89.4% and 10.6% respectively. The distribution of hepatic arteries was showed as typical and variant in 72.8% and 27. 2% respectively. The angle between proper hepatic and common hepatic arteries was obtuse in 62.4 % , right in 13.0 % , and acute in 24.6 % . The angle between gastroduode-nal and common hepatic arteries was obtuse in 82.4 % , right in 6.0 % , and acute in 11.6 % . The common styles of splenic artery were convex, horizontal, transverse S-shaped, concave, etc. The circle-and ripple-style were rarely seen in 6.7%. Conclusion; To study natural morphology of angiography of celiac artery and its branches can ensure the accuracy of catheterization and efficacy of the intervention therapies of hepatic,biliary,pancreatic,splenic,and gastric diseases.
