利凡诺与米非司酮联合应用在中孕引产中的疗效观察

目的：探讨利凡诺与米非司酮联合应用在中孕引产中的临床疗效与安全性,为治疗方案制定提供一定的依据。方法：选取本院妇科收治的妊娠14～27周要求引产的82例患者,随机将其分为观察组和对照组,每组41例。观察组采用口服米非司酮联合利凡诺羊膜腔内注射进行引产,对照组则单纯采用利凡诺羊膜腔内注射进行引产。对比两组患者引产过程中以及引产后各项指标的情况,对比两组患者宫颈成熟情况。结果：观察组腔内注射到宫缩以及排胎开始的时间、总产程时间均明显少于对照组。观察组的所有患者在第一次利凡诺羊膜腔内注射后,均出现宫缩,娩出胎儿,引产成功率为100％。对照组的有1例患者在第一次利凡诺羊膜腔内注射后72h没有出现宫缩,给予第二次利凡诺羊膜腔内注射后才成功娩出胎儿,引产成功率为97．56％。观察组引产后的阴道失血量（82．79±34．23）ml,明显少于对照组的（165．73±31．46）ml;观察组胎盘滞留粘连的发生率为4．88％,明显低于对照组的12．20％;观察组宫颈成熟率为92．68％,明显高于对照组的17．07％,差异均有统计学意义（P〈0．05）。结论：利凡诺联合米非司酮进行中孕引产,能够有效缩短患者的产程,减轻患者的痛苦,提高临床疗效,减少并发症的发生,值得进一步推广应用。

The Curative Effect of Rivanol Combined with Mifepristone in the Treatment of the Termination of Second Trimester Pregnancy

Objective： To investigate the clinical efficacy and safety of rivanol combined with mifepristone in the treatment of the termination of second trimester pregnancy, and to provide a basis for treatment plan.Method： 82 patients with 14-27 weeks fetus were selected in our hospital. They were randomly divided into the observation group and the control group, with 41 cases in each group.The patients in the observation group were accepted combination of rivanol intra-amniotic injection and mifepristone for induction of labor, while in the the control group only received rivanol intra- amniotic injection. The indexes of labor process and after induction of labor were compared.Result： The time of injection into the uterine cavity, starting fetal, total labor time of the observation group were significantly less than that in the control group.The successful rate of induced labor was 100% in the observation group, while it was 97.56% in the control group.The vaginal bleeding was （ 82.79 ± 34.23 ） ml in the observation group after the induction, it was significantly less than that in the control group （ 165.73± 31.46 ） ml; the retained placenta adhesion rate was 4.88% in the observation group, it was significantly lower than that in the control group （ 12.20% ） ; the cervical ripening rate was 92.68% in the observation group, it was significantly higher than that in the control group （ 17.07% ） ; the differences showed statistical significance （ P〈0.05 ） .Conclusion： Mifepristone combined with rivanol in the treatment of pregnancy can effectively shorten the patient＇ s birth process, reduce the suffering of patients, improve the curative effect, reduce complications, and is worth further application.
