摘    要:目的探讨脓毒性休克患儿液体复苏的临床意义。方法选择符合诊断标准的72例感染性休克患儿作为研究对象,其中好转存活36例,死亡36例;利用SPSS15.0的logistic回归分析对两组的收缩压、舒张压、平均动脉压、pH和入院24h尿量进行分析。结果感染性休克的病死率高达50%,死亡组的收缩压、舒张压、平均动脉压、pH和入院24h尿量与存活组比较差异有统计学意义。logistic回归分析中,模型检验X2=26.781,p=0.000,模型有统计意义。入院24h尿量、平均动脉压是危险因素(OR值、95%可信区间分别为5.197,1.359~19.878;1.125,1.025~1.234)。结论早期液体复苏能有效地改善循环与组织灌注,是脓毒性休克患儿抢救成功与否的重要因素之一。

Abstract:Objective To analysis the clinical significance of fluid resuscitation in patients with septic shock. Methods 72 cases were collected with diagnostic criteria of septic shock, which include 36 survival cases as survival group and 36 dead cases as death group. Analysis client data of these between two groups in SBP, DBP, MAP, blood glucose, pH, serum cholesterin, serum albumin,24-hour urine by Logistic Regression of SPSS 15.0. Result The fatality rate was 50%, which still stayed a high level. A regression model (X2=26.781 ,p=0.000) was established by Logistic Regression. There were statistical significance between two groups in SBP, DBP, MAP, pH, 24-hour urine. Among them, 2 items were selected as risk factors: 24-hour urine (OR:5.197,95% Ch 1.359- 19.878), MAP (OR:I. 125, 95% CI:1.025- 1.234). Conclusion Early fluid resuscitation could impove the microcirculation and regional perfusion in septic shock~ thus, it is oen of the main factors in influencing the prognosis of septic shock.
