特殊类型肾上腺嗜铬细胞瘤的超声诊断与临床研究

目的探讨超声对特殊类型肾上腺嗜铬细胞瘤的诊断价值,指使临床上对该病特殊性有更深入地了解,以便处理时采取有效对策.方法从收治36例嗜铬细胞瘤患者中,对其中11例特殊类型肾上腺嗜铬细胞瘤的临床特点及超声资料进行回顾性分析,并将声像图表现与病理结果对照.结果 4例为家族性(其中2例为双侧);1例伴有对侧肾上腺皮质腺瘤;4例为功能静止型;1例伴有顽固性低血钾;1例为双侧肾上腺区域多发性肿瘤(其中左侧子瘤3个,右侧子瘤2个).超声诊断符合率86.67%(13/15),超声对其定位符合率100%(15/15处),超声对肿瘤个数检出率83.33%(15/18个).结论超声显像对特殊类型肾上腺嗜铬细胞瘤的诊断具有重要的临床价值.但应指出,并发症与肿瘤个数检出的准确性尚应进一步提高.

Ultrasonographic Diagnosis and Its Clinical Research of Special Adrenal Pheochromocytoma

Objective To evaluate ultrasonographic diagnostic value for special adrenal pheochromocytoma in order to further recognize the speciality of the disease and to make effective remedies.Methods Among admitted thirty six patients with pheochromocytoma,we found eleven cases with special adrenal pheochromocytoma Their clinical features and ultrasonic documents were retrospectively analysed,and compared the ultrasonographic features with operation and pathology.Results Four cases were familiar(2 with tumors in both sides); 1 case with pheochromocytoma on one side and with aldosteronoma on the another side; 4 cases with static functioning pheochromocytoma and 1 case with multitumors in both sides (3 small tumors in the left,2 small tumors in the right) Ultrasonographic coincidence rate was 86 67%(13/15),Accurate site correlation rate was 100%(15/15),Ultrasonographic check up rate for tumorous numbers was 83 33%(15/18).Conclusions It is suggested that ultrasonic imaging provides important valuable information for diagnosis of special adrenal pheochromocytoma,but the complications and the diagnostic accuracy of tumorous numbers need further improvement
