子宫全切除术中应用腰-硬联合麻醉的临床价值分析

目的观察腰－硬联合麻醉在子宫全切除术中的应用效果。方法选择2011年5月～2012年12月本院收治的74例子宫全切除患者，将所有患者随机分为对照组与观察组各37例，对照组患者采用连续硬膜外麻醉，观察组患者采用腰一硬联合麻醉。比较两组患者的麻醉用药剂量，阻滞起效、完善时间．血流动力学情况以及临床不良反应的发生情况。结果对照组患者的有效率明显高于观察组（P〈0．05），观察组患者的麻醉阻滞起效时间和组织完善的时间都明显短于对照组（P〈0．05）。结论腰一硬联合麻醉的麻醉方式明确、效果显著，适用于子宫全切除术患者，值得在临床应用中广泛推广。

Clinical value analysis of application of combined spinal-epidural anes- thesia during total hysterectomy

Objective To observe the application effects of combined spinal-epidural anesthesia during total hysterectomy. Methods 74 patients with total hysterectomy enrolled and cured in our hospital from May of 2011 and December of 2012 were selected and analyzed retrospectively.All the patients were randomly divided into the control group and the observation group with 37 patients in each group,in which a continuous epidural anesthesia method was used in the control patients during treatments,and a combined spinal-epidural anesthesia method was employed in the patients of the observation group.Anesthesia drug doses,blocking onset effects,improvement times,hemodynamics and occurrences of clinical adverse reactions were compared in the patients from the two groups. Results Patients with ad- verse reactions in the control group were more than that of the observation group （P〈0.05）.The blocking onset time of anesthesia and tissue improvement time in the observation group were obvious earlier than that of the control group（P〈 0.05）. Conclusion The anesthesia method of the combined spinal-epidural anesthesia is clear, the treatment effect is significant,and it is applicable in patients with total hysterectomy,so it is worth popularizing widely in clinical applica- tions.
