肥胖儿童骨密度及胰岛素抵抗水平研究

摘    要:目的探讨儿童肥胖与骨密度及胰岛素抵抗的关系。方法选取7~14岁肥胖儿童60例及正常体重儿童60例,使用超声骨密度仪测量桡骨远端超声波速度(SOS),并抽血检测空腹血糖及胰岛素水平,采用胰岛素抵抗指数(HOMA—IR)评估胰岛素抵抗水平,比较两组骨密度及胰岛素抵抗水平的差异。结果肥胖儿童身高、体重、体重指数、腰臀比均显著高于正常儿童(P〈0.01);肥胖儿童SOS高于正常儿童,差异有统计学意义(P〈0.05);肥胖儿童HOMA—IR显著高于正常儿童(P〈0.01)。结论肥胖儿童骨密度及胰岛素抵抗水平高于正常体重儿童,减轻体重有利于改善胰岛素抵抗,但须注意科学方式,避免骨量减少。

Abstract:Objective To discuss the relationship among childhood obesity, bone mineral density (BMD), and insulin resistance. Methods 60 obese children aged 7-14 and 60 children with normal weight were selected. The ultrasound speed (SOS) of distal radius was measured by ultrasound bone densitometer. Fasting blood-glucose (FBG) and insulin were tested by drawing blood. The level of insulin resistance was evaluated by homeostasis model assessment of insulin resistance (HOMA-IR). All the differences between the two groups in BMD and level of insulin resistance were analyzed.Results The height, body weight, body mass index (BMI), and waist hip ration in obese children group were all remarkably higher than those in normal weight group (P〈0.01). In obese children group, the SOS and HOMA-IR were both greatly higher than those in normal weight group, which displayed statistical differences (P〈0.01). Conclusion The BMD and insulin resistance level in obese children are beth higher compared with those in normal weight children.Weight loss is beneficial for improvement of insulin resistance, but scientific method is necessary or it will lead to osteopenia.
