骨折后骨不连治疗分析

目的总结陈旧性骨折后骨不连的治疗经验,分析骨不连的病因,评价手术治疗的有效性。方法回顾性对济源钢铁（集团）有限公司职工医院骨科从2010年7月—2013年7月手术治疗的14例骨不连患者进行分析,均采用锁定加压钢板加自体骨移植的治疗方法。综合评价骨折骨折愈合率、手术并发症、临床效果。结果本组14例患者,经过术后随访,患者均达到骨性愈合。随访时间3～24个月,骨折愈合时间4～12个月,平均7.1个月。其中12例临近关节功能得到明显改善,另有2例临近关节仍存在活动障碍,无内固定失败等。结论骨不连是骨折切开复位内固定术后常见并发症,骨折部位、骨折类型的不同以及术后发生感染均是引起骨折后骨不连的重要原因。锁定加压钢板内固定加自体骨移植技术是治疗陈旧性骨不连较为有效的方法。

Clinical analysis of treatment of nonunion after fracture

Objective To summarize the experience of treatment of nonunion of old fracture,analysis of the causes of nonunion,evaluation of the effectiveness of operation treatment. Methods Retrospective review of 14 patients with bone in this hospital from July 2010- July 2013 operation treatment of nonunion patients were analysed,using method of treatment of locking compression plate combined with autogenous bone transplantation. Comprehensive evaluation of fracture healing rate,operation complications,clinical effect. Results In 14 cases,the postoperative follow- up,all patients obtained bone union. Follow up time was 3 to 24 months,fracture healing time was 4 to 12 months,average 7. 1months. Among the 12 cases of adjacent joint function improved,and another 2 cases still exist near the joint movement disorder,no internal fixation failure. Conclusion Bone nonunion is after open reduction and internal fixation of a common complication of fracture,the fracture site,fracture type and different postoperative infection is an important cause of bone nonunion after fracture. Locking compression plate fixation and autogenous bone graft is the treatment of old nonunion effective method.
