半导体红外线激光经巩膜行睫状体光凝术治疗难治性青光眼临床报告

摘    要:
目的 :观察 810nm半导体红外线激光经巩膜睫状体光凝术 (transcleraciliaryphotocoagnlationTSCPC)治疗难治性青光眼的临床效果。方法 :应用法国BVI公司生产的 810nm半导体红外线激光仪 ,经光纤将能量传输到激光头 ,在距角膜缘后 1.2~ 1.5mm ,紧贴眼球壁 ,使激光头与视轴平行 ,设置激光能量 1.8~ 2 .2W ,时间 2~ 3s,发射激光后听到眼内“爆破声”作为治疗能量的阈值 ,间隔 2mm进行一个光凝点 ,共 18~ 2 0个点 ,术后一月和二月观察眼压等变化情况。结果 :本组 6 2例患者术前眼压难以用手术或药物控制 ,范围在 38~ 85mmHg ,平均 (4 6 .2± 11.76 )mmHg ,经治疗后一月内眼压有 6 7.74%控制在 30mmHg以下 ,二月内有 93.5 5 %眼压控制在 30mmHg以下 ,其中 75 .81%控制在正常范围。结论 :810nm半导体红外线激光行TSCPC治疗难治性青光眼是一种安全有效 ,操作简单 ,易于掌握 ,患者乐于接受的治疗方法

The Clinical Report of Treating Refractory Glaucoma with 810 nm Infrared Laser Transdera Ciliary Photocoagulation
Abstract:
Objective To observe the clinical effects of treating refractory glaucoma with the 810 nm laser TSCPC.Methods Setting up the maximal power of 1.8~2.2 W and 2.0~3.0 sceonds,810 nm infrared ray laser,emitted by the laser(TWIN,BVI Company,France),was fibroptically transmitted to a probe of the laser which was localized in touch to the eyeball at 1.2~1.5 mm behine corneal limbus and paralled to the optic axis.The threshold of treating energy were determinated by hearing the "burst" sound from the eye.The points of photocoagulation had a interval of 2 mm each other and the numbers of it were sum of 18~20.The changes in intraocular pressure were measured one month and two months after the treatment.Results Intraocular pressure of 62 eyes in the 51 patients with refractory glaucoma(without controlled with operation or drugs),were measured in the range from 38 to 85 mmHg(46.20±11.76 X±SD) before the laser TSCPC.One month follow the laser photocoagulation treatment,42 out of 62 eyes (47.47%) have the decreased intraocular pressure being less than 30 mmHg.Two month after the treatment 58 out of 62 eyes (93.55%) being below 30 mmHg,and amongthem 47 eyes (75.81%) being under 20 mmHg,that is,normal range.Conclusion 810 nm semiconductor infrared ray laser treated refractory glaucoma through TSCPC is a safely,effectively and pimply and thus,the patients are preferably to accept it.
