续骨活血汤联合溶栓对老年四肢创伤骨折术后下肢血栓风险的影响

目的研究续骨活血汤联合溶栓对老年四肢创伤骨折患者术后双下肢静脉血栓风险的影响。方法选取2018年5月—2018年11月收治的老年四肢创伤骨折术患者100例为研究对象,按照数字表随机分组的方案分为治疗组50例,对照组50例。对照组术后2周采用利伐沙班干预,治疗组在对照组基础上联合续骨活血汤干预。观察两组干预两周后双下肢静脉血栓发生率,记录骨痂、疼痛评分,并比较前臂、小腿周径变化及血浆中D二聚体含量变化,另观察两组患者治疗期间不良反应发生率。结果两组患者治疗1周、2周后疼痛评分与治疗前比较均呈现降低趋势(P<0.05);且治疗组治疗1周、2周后,疼痛评分低于对照组,差异有统计学意义(P<0.05)。治疗1周、2周后两组患者前臂、小腿周径与治疗前比较均呈降低趋势(P<0.05);且治疗组治疗1周、2周后前臂、小腿周径显著低于对照组,差异有统计学意义(P<0.05)。治疗1周、2周后两组患者血浆中D二聚体含量与治疗前比较呈下降趋势(P<0.05);且治疗组治疗1周、2周后血浆中D二聚体含量低于对照组,差异有统计学意义(P<0.05)。治疗组不良反应发生率为2.00%与对照组2.00%相当,两组比较差异无统计学意义(P>0.05)。结论续骨活血汤联合溶栓可降低老年四肢创伤骨折患者术后双下肢静脉血栓风险,安全性高,具有较高的临床应用价值。

Effect of Xugu Huoxue Decoction combined with thrombolysis on lower limb thrombosis in elderly patients with traumatic fracture of extremities after operation

Objective To investigate the effect of Xugu Huoxue Decoction combined with thrombolysis on venous thrombosis of lower limbs in elderly patients with traumatic fracture of limbs after operation.Methods A total of 100 elderly patients with traumatic fracture of extremities admitted to our hospital from May,2018 to November,2018 were included and randomized into the observation group and the control group by using random number table(50 cases per group).The control group was treated with rivaroxaban two weeks after operation,while the observation group was treated with Xugu Huoxue Decoction combined with rivaroxaban.The incidence of venous thrombosis in the lower extremities in both groups was observed after two weeks of intervention.Scores of callus and pain were recorded,and changes of circumference of the forearm and leg,and plasma D dimer content,and the adverse reactions in both groups were observed and compared.Results The pain scores in both groups decreased after treatment(P<0.05),the pain score of the observation group was lower than that of the control group one and two weeks after treatment,with significant difference(P<0.05).One and two weeks after treatment,the circumference of forearm and leg in both groups decreased(P<0.05),the circumference of forearm and leg in the observation group was significantly lower than that in the control group,with significant difference(P<0.05).The plasma D-dimer content in the two groups decreased one and two weeks after treatment(P<0.05),the plasma D-dimer content in the observation group was lower than that in the control group,with significant difference(P<0.05).The incidence of adverse reactions in the observation group was 2.00%and that in the control group was 2.00%,without statistical difference between the two groups(P>0.05).Conclusion Xugu Huoxue Decoction combined with thrombolysis can reduce the risk of venous thrombosis in lower limbs of elderly patients with traumatic fracture of extremities after operation,which is safe and worthy of application.
