| 摘 要： | 目的 应用定量组织速度成像技术(QTVI)检测高血压患者左心室功能的改变,并评价肾素-血管紧张素系统(RAS)抑制剂对高血压患者左室功能的影响.方法 选择高血压患者68例,按是否服用RAS抑制剂再分为RAS抑制组及非RAS抑制组,并设立健康对照组.测量二尖瓣环室间隔侧和左室侧壁侧QTVI曲线上心室收缩期速度峰值 (Sa)、心室舒张早期速度峰值 (Ea) 、心室舒张晚期速度峰值 (Aa),并计算左室平均峰值.结果 高血压组的Sa波峰值、Ea波峰值及 Ea/Aa比值均低于正常对照组(P＜0.01);RAS抑制组的Sa波峰值(6.28±1.28)cm/s与正常对照组(6.64±1.63)cm/s比较差异无统计学意义,与非RAS抑制组(5.37±1.14)cm/s比较差异有统计学意义(P＜0.01).结论 抑制RAS能阻止或逆转高血压所致的左心室收缩功能损害;QTVI能在常规超声心动图指标异常之前检测到高血压患者的左心室收缩功能受损.
