原发性肝内周围型胆管细胞癌的CT表现

目的分析原发性肝内周围型胆管细胞癌(PIHPCC)的CT表现。方法回顾性分析1992—2005年经病理证实15例PIHPCC的CT平扫和强化表现。结果15例中肝左叶10例,右叶5例;单发12例,多发3例;病变直径3~12cm;圆形6例,分叶形9例,平扫低于肝实质,CT值8~29HU,7例见扩张胆管,1例钙化,2例结石;病变区轮廓膨突8例,萎陷7例。CT强化早期见周围不完全、内部条网状强化,延迟扫描强化显著。结论CT检查对PIHPCC诊断有重要作用。

CT Findings of Primary Intrahepatic Peripheral Cholangiocarcinoma

Objective To study the CT findings of primary intrahepatic peripheral cholangiocarcinoma(PIHPCC).Methods Plain CT and contrast-enhanced CT findings of PIHPCC confirmed pathologically in 15 cases in our hospitals from 1992 to 2005 were retrospectivelyanalysed.Results Among 15 cases,there were 10 cases in left liver and 5 cases in right liver.A single mass was found in 12 cases,andmultiple masses in 3 cases.The masses were 3~12 cm in diameter.The mass shape was round in 6 cases,and lobulate in 9 cases.On routine CT scan , the masses were low density (8~29 HU ) , with dilated bile ducts in 7 cases , calcification in 1 cases and cholelith in 2 cases.The hepatic margin was protrusion at the area of lesions in 8 cases and was atrophy in 7 cases.On contrast-enhanced CT,a peripheral,discontinuous slight enhancement was found on arteral phase,with some stripe and network enhancement on interal region,and obviousenhancement on delay study.Conclusion CT study is of important rule in diagnosis of PIHPCC.
