组织多普勒超声结合Tei指数评价心肌梗死患者右心室功能

摘    要:目的:应用心肌组织多普勤(TDI)超声及其测定的Tei指数综合评价急性下壁及前壁心肌梗死(MI)患者右心室功能。方法:正常对照组20例,急性下壁MI20例,急性前壁MI20例。在标准心尖四腔心观二维图像指引下,采用TDI频谱多普勒技术记录三尖瓣环右室游离壁处速度曲线,测量计算Tei指数并测量收缩期、舒张早晚期的最大运动速度。结果:与对照组相比,急性心肌梗死患者三尖瓣环右室游离壁处下壁和前壁Tei的指数均显著升高(P<0.01);两者舒张早期速度均显著降低,舒张早期与晚期速度比值也明显降低(P<0.05);对于最大收缩速度,在下壁MI组显著降低(P<0.01),在前壁MI组无下降(P>0.05)。结论:TDI频谱多普勤技术能反映急性MI导致的右室舒缩功能减低,结合Tei指数可以更好、更全面地评价MI后右室整体功能。

Abstract:Objective:To evaluate right ventricular(RV)function of acute inferior and anterior myocardial infarction(MI)with tissue Doppler imaging and Tei index.Methods:Twenty patients with acute inferior and 20 patients with acute anterior MI were prospectively compared with 20 age 2 matched healthy individuals.From the echocardiographic apical 4-chamber views,peak systolic and early and late diastolic velocities of the tricuspid annulus was recorded at the RV free wall by tissue Doppler imaging.The Tei index by TDI were calculated respectively at the same site.Results:When compared with control group,Tei index of RV increased in both inferior and anterior MI(P<0.05,P<0.01),the peak systolic velocity of the tricuspid annulus was reduced in inferior MI(P<0.01),but was not reduced in anterior MI(P>0.05),the peak early diastolic velocity and the ratio of peak early and late diastolic velocities at the above site was reduced in both.Conclusion:Tissue Doppler imaging combined with Tei index can show the impaired RV function in association with the inferior and anterior MI,it may become a new technology to assess RV function.
