超声刀在腹腔镜外科手术中的应用

目的 探讨超声刀在腹腔镜外科手术中的优缺点及其应用前景。方法 使用超声刀代替电刀行腹腔镜肠粘连松解、阑尾切除、结直肠切除、胆囊切除、疝修补、胆总管切开探查、脾切除、胃大部分切除等手术。结果 52例12种手术全部顺利完成,无中转开腹。手术时间12～300分钟。其中42例病人完全使用超声刀完成手术,手术出血0-100毫升。无术后继发出血。结论 超声刀凝血功能强,术中出血减少,使腹腔镜手术中大网膜和肠系膜的处理及粘连的分离变得简单容易,术野清晰,无电流通过人体使手术更安全,切割器的用量减少从而降低手术费用。

The use of Ultracision - Harmonic scalpel in Laparoscopic operation

Objective In order to evaluate the advantage and shortcoming of Ultracision - Harmonic Scalpel (UHS) in Laparoscopic Operation. Methods 51 cases of Laparoscopic procedures had been done by UHS. Patient's age ranged from 8-78 years, 21 male and 30 female. The procedures involved in right hemicolectomy(n = 6) , left hemicolectomy ( n = 1) , miles' procedures ( n = 1) , cholecystectomy ( n = 11), appendectomy ( n = 18), enterolysis ( n = 6) , herniorrhaphy(n = 2) , choledocholithotomy(n = 2) , operation for extra - uterine gestation (n = 3) , gastrectomy(n = 1) , splenectomy(n = 1) . Results All cases were successfully done. The average lengths of operative stay were 12 ~ 300min. The average bleeding was 0 - 100ml. No patient required conversion to open laparotomy. There was no complication . Conclusion Based on these preliminary results, we beliveve that UHS is a safely and effectively tool in Laparoscopic Surgery.
