彩色多普勒超声在检测新生儿窒息多脏器血流动力学中的应用

摘    要:目的 研究新生儿窒息后多脏器血流动力学变化情况。方法 选取窒息新生儿112例作为观察组,正常新生儿20例作为对照组。采用彩色多普勒超声,分别测定新生儿大脑中动脉(MCA)、肾动脉(RA)、腹腔动脉(CA)以及肠系膜上动脉(SMA)的血流动力学各项参数。结果 新生儿窒息后多脏器血流速度均减慢,舒张期更为明显,且重于大脑器官。各脏器血流速度减慢程度与缺氧程度高度相关(r=0.92),其它各血流参数也均低于对照组。结论 血流量减少是新生儿窒息后多脏器损伤和功能障碍的主要原因。

Abstract:Objective To study multiorganic hemodynamic changes of neonates with asphyxia.Methods 112 asphyxial neonates and 20 normal neonates (control group) were included into the study.The hemodynamic parameters of the middle cerebral artery(MCA),renal artery (RA),coeliac artery (CA),and superior mesenteric artery (SMA) were measured by color Doppler ultrasound.Result The blood flow velocity of these arteries were slower in asphyxial neonates than in normal neonates,especially in diastolic period.The changes of the velocity of all other organs were more severe than that of brain.Moreover,the degree of these changes was found to be strongly related with the severity of anoxia (r=0.92).All of the hemodynamic parameters of asphyxial neonates were significantly lower than that of control group.Conclusions The decrease of blood flow volume may be the main reason of multiorganic injury and dysfunction in neonates with asphyxia.
