医源性主干静脉损伤20例原因分析与防治

目的：探讨医源性主干静脉损伤原因及其防治措施。方法：回顾性分析20例医源性主干静脉损伤的类型、原因，为制定防治措施提供依据。结果：主干静脉损伤发生率占同期主干血管损伤的80％，这与静脉本身的特点及其疾病的解剖有关。术中解剖层次不清、操作不规范是造成损伤的主要原因。结论：术前充分估计病变组织与周围主干静脉的毗邻关系，术中谨慎、规范操作，明晰解剖关系与层次是降低主干静脉损伤的关键。

The Causes and Prevention of Iatrogenic Major Vein Injury

Objective To explore the reason and prevention of iatrogenic major vein injury. Method Review and analyses of 20 cases of iatrogenic major vein injuries. Results The rate of major vein injury was higher than that of major artery during the operations. The main causes were improper technical maneuver and unclear anatomical dissection. Conclusions In order to reduce the occurrence of iatragenic major vein injury, the surgeon must give full estimation to the anatomical relation between the lesion and the adjacent major vein before the operation. Standarized technic and clear anatomical dissection during the operations are important to the prevention of iatrogenic major vein injury.
