脑膜血管型梅毒的临床及影像特点

目的分析脑膜血管型梅毒的临床及影像特征以提供早期诊断依据。方法分析经临床及实验室确诊的2例脑膜血管型梅毒患者的有关临床资料。结果脑膜血管型梅毒早期以突然出现偏瘫、癫痫发作、失语、认知障碍为主要临床表现,影像上主要为病灶散在多发,头颅MRA或CTA可见颅内血管狭窄,实验室检查有特征性改变。结论脑膜血管型梅毒早期临床容易误诊,应重视梅毒血清和脑脊液抗体的检查,结合头颅MRI及MRA或CTA可以诊断,大剂量足疗程青霉素治疗能取得显著疗效。

The clinical and imagine features of meningovascucular syphilis

Objective To analyse the clinical and imagine features of meningovascucular syphilis so as to make an early diagnosis available.Methods The clinical and laboratory data of the definite two case of meningovascucular syphilis was analysed by a retrospective method.Results The early chief manifestations of hemiplegia,aphasia,mental confusion,seizure.With intracranial vascular narrowing in magnetic resonance angiography(MRA) or computed tomography angiography(CTA),as well as characteristic changes in laboratory examination.Conclusion We shou pay more attention to the examination of serum and cerebrospinal flui(csf) antibody against syphilis because of the high misdiagnosis rate at early stage.Combined with the help of MRI + MRA or CTA,meningovascucular syphilis could be early diagnosed.With a large dose and sufficient courses of penicillin treatment.meningovascucular syphilis may hopefully get predominant effects.
