准分子激光原位角膜磨镶术相关角膜瓣并发症分析

目的探讨准分子激光原位角膜磨镶术（LASIK）相关的角膜瓣并发症。方法回顾分析实施LASIK手术的2340例（4608眼）近视眼患者相关角膜瓣并发症的发生、影响因素及处理措施。结果LASIK相关角膜瓣并发症的发生率为1.04%（48/4608）。瓣下感染3例,占0.06%（3/4608）,发生于术后前3天2例,术后13天1例;角膜瓣ButtonHole 8例,占0.17%（8/4608）;角膜瓣明显偏位1例,占0.02%（1/4608）;游离瓣11例,占0.24%（11/4608）;术后早期角膜瓣移位、角膜瓣皱褶及瓣下异物19例,占0.41%（19/4608）;角膜瓣下上皮植入4例,占0.09%（4/4608）;术后远期外伤至角膜瓣移位2例,占0.04%（2/4608）。结论LASIK相关角膜瓣并发症的发生率较低,严格的手术操作、良好的设备可进一步减少并发症的发生。科学的处理,可获得良好的手术效果。

Analysis on flap related complications of laser in situ keratomeliusis

Objective: To investigate the flap related complications of laser in situ keratomeliusis(LASIK).Methods: The flap complications of LASIK were retrospectively analyzed in 4608 eyes of 2340 patients.Their affecting factors and treatment measures were proposed.Results: The overall rate of the flap related complications was 48 in 4608(1.04%),infection 3 in 4608(0.06%),flap buttonhole 8 in 4608(0.17%),decentral flap 1 in 4608(0.02%),free cap 11 in 4608(0.24%),corneal flap folds and foreign body under the flap 19 in 4608(0.41%),epithelial implantation 4 in 4608(0.87%),and traumatic displaced flap 2 in 4608(0.04%).Conclusion: The incidence of the flap related complications is very low.Strict operation and nice facilities can decrease its occurrence.The clinical results are very satisfied if these complications are treated scientifically.
