玻璃体切割术后角膜内皮细胞变化的临床相关研究

目的 探讨玻璃体切割术后相关因素对角膜内皮细胞的影响.方法 将没有角膜内皮原发病变需要行玻璃体手术的294例患者依据其年龄、是否保留晶状体及填充物不同进行分层分组,同一患者健眼为对照组.用角膜内皮镜或共焦显微镜观察术后1周、1、3、6、12、24个月角膜内皮细胞的变化,并对相关指标进行比较.结果 不同年龄、是否保留晶状体、不同的填充物对术后各时间的角膜内皮细胞的密度没有影响(P＞0.05)；不同年龄、不保留晶状体者术后填充硅油及20％C3 F8气体,其术后角膜内皮细胞、AVE、CV及RHC发生变化(P＜0.05).结论 玻璃体切割术后填充硅油或C3F8,保留晶状体有助于减轻对角膜内皮细胞的损伤.

THE CLINICAL ANALYSIS OF THE CHANGES IN CORNEAL ENDOTHELIAL CELL AFTER VITRECTOMY

Objective To investigate the effect of the postoperative vitrectomy-related factors on corneal endothelial cells.Methods294 cases of corneal endothelial primary diseases requiring vitreous surgery were divided into different groups according to their age,whether to retain lens and the different fillers in the vitreous cavity,and the other healthy eye of each patient was used to form the control group.The corneal endothelial cell changes were respectively measured with corneal endothelial microscopy or confocal microscopy post-operatively in 1 week and 1,3,6,12 and 24 months later.The related indexes were compared.ResultsThe age,whether to retain lens and the different fillers in the vitreous cavity produced no effects on corneal endothelial cell density（P> 0.05）.Different age,not retaining lens and filling silicone oil and 20% C₃F₈ could change the corneal endothelial cell、AVE、CV and RHC after vitrectomy（P<0.05）.Conclusion Retaining the lens and filling silicone oil or 20% C₃F₈ after vitrectomy can help reduce the damage to corneal endothelial cell in the eyes.
